摘要
【目的】比较锁定钢板与解剖钢板内固定治疗pilon骨折的疗效，并分析对踝关节功能的影响。【方法】选取80例pilon骨折患者作为研究对象，按手术方法不同分为对照组和观察组，每组40例。两组均接受切开复位内固定术，对照组采用解剖钢板内固定，观察组采用锁定钢板内固定。比较两组手术相关指标、并发症发生率、临床疗效和治疗前后踝关节功能相关指标。【结果】观察组的手术时间、术中出血量、愈合时间和住院天数均明显少于对照组，差异均有统计学意义（P〈0．05）；观察组的术后并发症发生率明显少于对照组，差异具有统计学意义（P〈0．05）；观察组的治疗总有效率明显高于对照组，组间比较差异有显著性（P〈0．05）；两组治疗后踝穴的高度、宽度和深度差值均较治疗前明显降低，且观察组治疗后的各项指标值均明显少于对照组，差异均具有统计学意义（P〈0．05）。【结论】与解剖钢板相比，锁定钢板内固定治疗Pilon骨折对机体创伤更小，术后并发症更少，疗效更佳，踝关节功能恢复更好，值得临床推广应用。
[ObjectivelTo compare the clinical effect of lock plate and anatomical plate fixation in the treatment of Pilon fracture, and to analyze its influence on ankle function. [MethodslA total of 80 patients with Pilon fracture were selected as study subjects. They were divided into the control group and the observation group, with 40 cases in each group. All patients underwent open reduction and internal fixation. The control group was subjected to anatomic plate fixation, while the observation group received lock plate fixation. The surgical indicators, the incidence of com- plications, clinical curative effect and ankle joint function related index before and after the treatment of two groups were compared. [ResultslThe operation time, intraoperative blood loss, healing time and length of hospital stay in the observation group were less than those in the control group; the differences were statistically significant（P〈 0.05）.The postoperative complication rate of the observation group was obviously less than that of the control group; the difference was statistically significant（P〈0.05）. The total effective rate in the observation group was higher than that in the control group with statistical significance （P〈O.OS）.The different values of ankle height, width and depth of patients in the two groups after treatment were lower than those before treatment. In addition, these ankle joint function indicators in the observation group after treatment were significantly less than those in the control group, and the differences were statistically significant （P〈0,05）. [ConclusionlCompared to anatomic plate fixation, lock plate internal fixation in the treatment of Pilon fracture is less traumatic to the body, causes fewer postoperative complica- tions, has better efficacy, results in better recovery of ankle joint function, and is worthy of clinical application.
