连续性肾脏替代治疗在ICU重症合并多器官功能障碍中的临床应用 被引量:2

Clinical application of continuous renal replacement therapy in ICU patients with multiple organ dysfunction syndrome

摘要 目的分析研究对重症监护室重症合并多器官功能障碍(MODS)患者应用连续性肾脏替代治疗(CRRT)的临床效果。方法选取该院100例ICU重症合并多器官功能障碍患者,将其随机分为对照组和试验组,对照组采用间歇性血液透析治疗,试验组采用CRRT,对比两组患者的治疗效果。结果治疗前,两组患者平均动脉压(MAP)、心率(HR)和心脏指数(CI)水平比较差异均无统计学意义(P>0.05);治疗2周后,试验组患者MAP、HR和CI水平均显著优于对照组患者(P<0.05);两组患者治疗2周后MAP、HR和CI水平均显著优于同组患者治疗前(P<0.05);试验组患者治疗30 d后死亡率为38%(19/50),明显低于对照组患者的70%(35/50)。结论对ICU重症合并MODS患者使用CRRT疗法有利于维持患者血流动力学处于稳定状态,并为患者争取更多的治疗时间,促进患者疾病的康复,值得临床上进行深入的研究和应用。 【Objective】To evaluate the application of continuous renal replacement therapy(CRRTT) in patients with severe multiple organ dysfunction syndrome(MODS) in intensive care unit(ICU).【Methods】One hundred ICU patients with MODS in our hospital were selected, and were randomly divided into the control group and the experimental group, with 50 cases in each group. The control group was treated with intermittent hemodialysis, the experimental group was treated with CRRT, and the curative effect was compared between the two groups.【Results】Before treatment, the difference in the mean arterial pressure(MAP), heart rate(HR) and cardiac index(CI) between the two groups was not statistically significant(P〈0.05); after 2-week treatment, MAP, HR and CI in the experimental group were significantly superior to those in the control group(P〈0.05); after 2-week treatment, MAP, HR and CI in both two groups were significantly superior to those before treatment(P〈0.05); after 30-day treatment, the mortality rate in the experimental group was 38%(19/50), which was significantly lower than 70%(35/50) in the control group.【Conclusion】The use of CRRT therapy in ICU patients with severe MODS is beneficial to maintain hemodynamics in stable state, buy more treatment time for the patients and promote the rehabilitation of patients, it is worthy of in-depth clinical research and application.
