摘要
目的 研究亚胺培南耐药鲍曼不动杆菌的耐药性特点以及分析碳青霉烯酶相关的耐药基因型。 方法 选取2016年6月-2017年6月在我院进行了亚胺培南耐药鲍曼不动杆菌分离操作的菌株40例;对其使用VITEK-2 compact的方法进行全自动微生物鉴定以及药敏测试;通过检测数据分析研究亚胺培南耐药鲍曼不动杆菌的耐药性特点,并检测其最小抑菌浓度;同时对已分离完毕的40株亚胺培南耐药鲍曼不动杆菌菌株使用基因扩增仪对其16S rRNA甲基化酶耐药基因以及碳青霉烯酶进行扩增测序。 结果 在40株亚胺培南耐药鲍曼不动杆菌菌株中:其中有38株菌株携带OXA-23型基因,且这38株菌株携带OXA-23型基因中均发现其上游序列ISAbal的存在。 结论 导致鲍曼不动杆菌对亚胺培南耐药性不断增长的主要原因是由带有OXA-23型基因的碳青霉烯酶而产生的;并且主导耐药性产生作用的物质是存在于上游序列的ISAbal。
Objective : to study the drug resistance characteristics of amine-resistant acinetobacter baumannii and to analyze the drug-resistant genotypes associated with carbapenase. Methods a total of 40 cases of impenem resistant acinetobacter baumannii isolated from our hospital from June 2016 to June 2017 were selected.The method of vitek-2 compact was used for automatic microbial identification and drug sensitivity test.The drug resistance characteristics of amine-resistant acinetobacter baumannii were studied by analyzing the test data,and the minimum antimicrobial concentration was detected.At the same time,40 strains of impenan resistant acinetobacter baumannii isolated were amplified and sequenced with gene amplification apparatus for its 16S rRNA methylase resistance genes and carbapenems. Results : in 40 strains of imine culture south resistant acinetobacter baumannii strains in:there are 38 strains carrying OXA - 23 genes,and 38 strains carry OXA - 23 genes are found in its upstream sequence ISAbal exists. Conclusion : the main reason for the increasing resistance of acinetobacter baumannii to imipenem is caused by carbapenase with oxa-23 gene.Moreover,the substance that plays a leading role in drug resistance is ISAbal,which exists in the upstream sequence.
