摘要
背景:溴结构域蛋白4(BRD4)已被证实参与了细胞恶性转化以及肿瘤细胞的侵袭、转移过程,但其在胃癌中的作用尚未见文献报道。目的:探讨BRD4、基质金属蛋白酶-13(MMP-13)在胃癌组织中的表达及其与胃癌临床病理特征和预后的关系。方法:收集进展期胃癌组织83例、早期胃癌组织45例、胃癌前病变组织42例和正常胃黏膜上皮组织38例,采用免疫组化方法检测BRD4、MMP-13表达,分析进展期胃癌组织中两者表达的相关性以及两者与肿瘤临床病理特征和患者预后的关系。结果:BRD4和MMP-13在进展期胃癌组织中的表达水平显著高于早期胃癌、癌前病变和正常胃黏膜上皮组织(P<0.05),两者表达水平呈显著正相关(r=0.258,P=0.019),且表达阳性者预后显著差于表达阴性者(P<0.05)。BRD4表达与进展期胃癌的淋巴结转移相关(P<0.05),多因素分析显示其为影响进展期胃癌患者预后的独立危险因素(OR=10.696,95%CI:1.137～100.609,P=0.038)。结论:胃癌组织中高表达的BRD4可能通过调控MMP-13信号通路促进胃癌恶性进展,影响患者预后。
Background： Bromodomain-containing protein 4（ BRD4） is proved to be a regulator of malignant transformation and involved in the invasion and metastasis of tumor cells. However,the roles of BRD4 in gastric cancer have not been reported yet. Aims： To explore the expressions of BRD4 and matrix metalloproteinase-13（ MMP-13） in gastric cancer tissue and the association with clinicopathological characteristics and prognosis. Methods： A total of 208 paraffin-embedded gastric tissues,including 83 advanced gastric cancer,45 early gastric cancer,42 precancerous lesions,and 38 normal gastric mucosa were collected and examined with immunohistochemistry for BRD4 and MMP-13 expression. Correlation between BRD4 and MMP-13,as well as their correlations with clinicopathological characteristics and prognosis of advanced gastric cancer were analyzed. Results： Expressions of BRD4 and MMP-13 were both significantly increased when compared with normal gastric mucosa,precancerous lesions,and early gastric cancer（ P 〈 0. 05）,and were associated with a poor survival rate in advanced gastric cancer（ P 〈 0. 05）. Expression of BRD4 was positively correlated with expression of MMP-13（ r = 0. 258,P = 0. 019） and lymph node metastasis（ P 〈 0. 05） in advanced gastric cancer. Furthermore,multivariate analysis identified BRD4 as an independent prognostic factor for advanced gastric cancer（ OR = 10. 696,95%CI： 1. 137-100. 609,P = 0. 038）. Conclusions： The highly expressed BRD4 in gastric cancer might promote tumor progression via MMP-13 signaling pathway and affect the prognosis of patients.
