高原地区藏、汉族动脉导管未闭合并重度肺动脉高压患者介入封堵术后肺动脉压比较

Comparation of pulmonary arterial pressure after transcatheter closure for patent ductus arteriosus associated with severe pulmonary hypertension between Tibetan and Han patients

摘要 目的:探讨高原地区藏族与汉族动脉导管未闭(PDA)合并重度肺动脉高压(SPH)患者介入封堵术后心脏功能及平均肺动脉压(mPAP)的变化。方法:将72例接受介入封堵治疗的PDA-SPH患者分为藏族组(34例)和汉族组(38例),分别在术前、术后即刻测定mPAP、平均主动脉压(MAP)、左室射血分数(LVEF)及左室容积,进行组间及组内比较。结果:术前两组患者上述指标差异无统计学意义;封堵术后,两组mPAP、左室容积、LVEF均较术前下降,且藏族组mPAP下降幅度更显著(P<0.05);两组术后MAP均较术前升高(均P<0.05);两组均无严重并发症发生。结论:高原地区PDA多合并SPH,积极行封堵治疗纠正畸形可有效缓解PAH,改善心脏功能,藏族即刻疗效优于汉族患者,可能与世居藏族的遗传优势及高原适应性有关。 Objective:To compare mean pulmonary arterial pressure(mPAP)and cardiac function after transcatheter closure for patent ductus arteriosus associated with severe pulmonary hypertension(SPH)between Tibetan and Han patients residing in Qinghai-Tibet plateau.Method:A total of 72 patients with PDA-SPH were included and divided into Tibetan group(n=34)and Han group(n=38).The mPAP,mean aortic pressure(MAP),left ventricle ejection fraction(LVEF)and left ventricular volumes were measured before and immediately after transcatheter closure.Result:All patients underwent successful transcatheter closure of PDA without major complications.Immediately after the procedure,the mPAP,left ventricular volumes and LVEF decreased significantly both in two groups(all P〈0.05),and the decrease of mPAP in Tibetan was more significant than that in Han(P〈0.05).The MAP increased in both two groups(both P〈0.05).Conclusion:Transcatheter closure of PDA-SPH is an effective method to decrease the mPAP and optimize the cardiac function for PDA-PAH patients in plateau.The immediately effects is better in Tibetan than that in Han.It may be caused by genetic advantage and high altitude adaption of Tibetans.
