摘要
目的探究在膝关节置换术(total knee arthroplasty,TKA)中行关节内不均衡截骨可纠正下肢力线的最大截骨角度。方法随机选取我院2015年7月~12月行膝关节置换术患者50例与膝关节核磁共振患者100例,分为:手术组(取术中测量数据);影像组(取MRI片上测量数据)。测量数据包括:内侧副韧带止点附着最低点到截骨面距离;外侧最低点距离;截骨面横径。通过三角函数Tan a/c、Tan b/c求得截骨角度,并对所得数据进行统计学分析。同时,通过查阅文献进行限定讨论以得到最终角度。结果内侧截骨角度为13. 194°±1. 623°,外侧截骨角度为12. 591°±1. 830°。结论最大截骨角度即为在TKA术中应用不均衡截骨术可纠正关节外畸形的最大角度,畸形角度在均值以下可考虑通过关节内截骨进行矫正,超出极值者应通过关节外截骨或其他方式进行矫正。
Objective To investigate the maximum osteotomy angle of correcting the lower ex- tremity force line in intra-articular uneven osteotomy during total knee arthroplasty (TKA). Methods Fifty patients with total knee arthroplasty and 100 clinical cases with knee-joint MRI in our hospital from July to December 2015 were randomly selected and divided into surgery group (data from operation) and image group (data from MRI). The measured data included: the distance from the lowest attachment point of medial collateral ligament to osteotomy section, the distance from the lowest lateral collateral ligament to osteotomy section, and the transverse diameter of osteotomy section. The osteotomy angle was obtained by Tan arc or Tan b/c, and statistical analysis was performed. At the same time, restricted discussion by referring to litera- ture were carried out to obtain the final angle. Results The mean value of medial osteotomy angle was ( 13. 194± 1. 623 ) °, while the mean value of lateral osteotomy angle was ( 12. 591 ±1. 830) °. Conclusion The maximum osteotomy angle is the maximum angle of correcting the lower extremity force line in intra-articular uneven osteotomy during TKA. The deformity angle which under the mean level may be considered to be corrected by interarticular osteotomy, while the deformity angle which beyond the mean level should be corrected by exarticular oste- otomy or otherwise.
