摘要
目的:探讨实时三维超声评价肥厚型心肌病(HCM)舒张失同步性及舒张功能。方法:将2015年10月至2017年10月在汕尾市人民医院心内科治疗的60例HCM患者作为观察组,将同期60例健康体检者作为对照组,均采用二维超声和实时三维超声检查,比较两组研究对象的左心室各参数、舒张功能、收缩功能。结果:观察组患者左室射血分数(LVEF)与对照组相比明显降低,而收缩末容积(ESV)、舒张末容积(EDV)、左室舒张早晚期血流速度峰值比值(E/A)、二尖瓣舒张早期血流速度峰值与二尖瓣环舒张早期运动速度峰值(E/Ea)明显升高,差异具有统计学意义(P<0.05);观察组患者舒张末期球形指数(EDSI)、左室舒张同步性指数(DDI)、平均舒张末时间(MED)、舒张末离散度(DISPED)与对照组相比明显升高,差异具有统计学意义(P<0.05);观察组收缩末期球形指数(ESSI)、左室收缩同步性指数(SDI)、平均收缩末时间(MES)、收缩末离散度(DISPES)与对照组相比也显著升高,差异具有统计学意义(P<0.05)。结论:实时三维超声评价HCM舒张失同步性及舒张功能效果显著,能够早期发现舒张功能不全影响收缩功能的同步性。
Objective To evaluate the diastolic and diastolic function of hypertrophic cardiomyopathy（HCM） in realtime three-dimensional ultrasound. Methods In October 2015 to October 2017 in Shanwei people＇s hospital heart treatment of 60 patients with hypertrophic cardiomyopathy patients as observation group, will be in the same period of 60 cases of healthy physical examination as a control, adopts two-dimensional ultrasound and real-time three-dimensional ultrasound examination, compare two groups of the parameters of the left ventricle, diastolic dysfunction, systolic function. Results The observation group LVEF,ESV,EDV, E/A, E/Ea were significantly lower than the control group, and the difference was statistically significant（P 0.05）. The observation group EDSI, DDI, MED and DISPED were significantly increased compared with the control group, and the difference was statistically significant（P 0.05）. The observation group ESSI, SDI, MES and DISPES also increased significantly compared with the control group, and the difference was statistically significant（P 0.05）. Conclusion Real-time threedimensional ultrasonography can be used to evaluate the diastolic and diastolic function of hypertrophic cardiomyopathy, and it can be found that the diastolic function does not affect the synchronicity of systolic function.
