脑梗死部位对运动执行网络功能连接的影响 被引量:1

The impact of different infarction regions on functional connectivity in motor execution network

摘要 目的探讨脑梗死部位对运动执行网络功能连接的影响。方法选择144例累及运动通路的脑梗死患者作为研究对象,其中基底节区脑梗死(位于基底节内囊水平,capsule stroke,CS)97例,脑桥梗死(pontine stroke,PS)47例;另外选择50例正常人作为对照组。所有被试均采用3.0T磁共振扫描仪进行3D-T1WI解剖像、静息态功能磁共振成像扫描,再进行静息态功能连接计算。比较不同梗死部位的患者在运动执行网络内功能连接的变化。结果左侧CS组:左侧背外侧运动前区(MNI坐标x,y,z:-22,-13,57),右侧初级运动皮层(MNI坐标x,y,z:38,-22,56)rsFC增高(F=3.951,P=0.022),事后检验显示,左侧CS患者与对照组(P=0.031)和左侧PS患者(P=1.000)相比rs-FC增高;右侧腹侧运动前区(MNI坐标x,y,z:53,0,25),左侧顶上小叶(MNI坐标x,y,z:-22,-64,54)rsFC增高(F=4.017,P=0.021);右侧中央后回(MNI坐标x,y,z:37,-34,53),右侧腹侧运动前区(MNI坐标x,y,z:53,0,25)rsFC减低(F=-3.788,P=0.025)。右侧CS组:右侧背外侧运动前区(MNI坐标x,y,z:28,-10,54),左侧初级运动皮层(MNI坐标x,y,z:-38,-22,56)rsFC增高(F=4.438,P=0.014);右侧腹侧运动前区(MNI坐标x,y,z:55,0,25)-右侧中央后回(MNI坐标x,y,z:37,-34,53)rsFC增高(F=4.830,P=0.010);左侧初级运动皮层(MNI坐标x,y,z:-38,-22,56)-左侧辅助运动区(MNI坐标x,y,z:-5,-4,57)rsFC减低(F=-5.102,P=0.007)。左侧PS组:右侧齿状核-左侧辅助运动区rsFC存在差异(F=4.939,P=0.009),左侧PS患者与对照组相比显示出rsFC增高(P=0.006);左侧齿状核-右侧辅助运动区rsFC增高(F=3.431,P=0.036);右侧小脑前叶下部-右侧背外侧运动前区rsFC减低(F=-4.114,P=0.019)。右 Objective To explore the influence of the location of the infarction lesion on the restingstate functional connectivity (rsFC) of the motor execution network. Methods A total of 144 patients with infarction of the motor pathway were involved in the study, including 97 patients with capsule stroke (CS) and 47 patients with pontine stroke (PS).50 age-matched healthy subjects were enrolled.After acquiring the structural images and the resting-state functional MRI data of all the subjects by 3.0-Tesla MR scanner, the functional connectivity was calculated in different regions of interest and the differences of the rsFC within the motor execution network among patients with different infarction location were compared. Results Left CS patients exhibited increased rsFC in L PMd (MNI x,y,z: -22,-13,57) -R_M1 (MNI x,y,z:38,-22,56) (F= 3.951 ,P= 0.022), the increased rsFC than healthy controls (P= 0.031 ) ; Left CS patients exhibited increased rsFC in R_PMv ( MNI x, y, z : 53,0,25 ) -LSPL ( MNI x, y, z : - 22, - 64,54) than healthy controls ( F= 4.017, P= 0.021 ), left CS patients exhibited decreased rsFC in R PCG ( MNI x, y, z : 37, - 34,53 ) -R_ PMv ( MNI x, y, z : 53,0,25) than healthy controls ( F= - 3.788, P= 0.025 ). Right CS patients exhibited increased rsFC than healthy controls in R_PMd( MNI x ,y, z :28,-10,54)-L_M1 (MNI x, y, z:-38,-22,56) (F= 4.438,P= 0.014) , right CS patients exhibited increased rsFC than healthy controls in R_PMv( MNI x, y, z : 53,0,25) -R_PCG( MNI x, y, z : 37, - 34,53 ) ( F= 4.830, P = 0.010), right CS patients exhibited decreased rsFC in L MI(MNI x,y,z:-38,-22,56)-L_SMA(MNI x,y,z:-5,-4,57) than healthy controls (F=-5.102,P= 0.007).And the left PS patients showed increased rsFC than healthy controls in R_DN -L_ SMA (F= 4.939,P=0.009) ,left PS patients exhibited increased rsFC than healthy controls in L_DN-R_SMA (F= 3.431 ,P=0.036) ,left PS patients exhibited reduced rsFC
