摘要
目的探讨血液病进行血液形态学检验、骨髓活检、免疫分型和细胞遗传学联合检查特点。方法该次研究对象共有100例,均是随机选取该院2015年4月—2017年12月所接收的血液病患者,让这100例患者首先接受单独的血液形态学检验,然后再接受多种诊断方式联合检验,对比单独血液形态学检验与多种诊断联合检验的检出率。结果在该次研究所选取的100例研究对象中,其中患有血小板减少症的患者共有40例,患有慢性粒细胞性白血病的患者共有20例,患有再生障碍性贫血的患者共有18例,患有慢性淋巴细胞白血病的患者共有16例,患有骨髓增生异常综合征的患者共有6例。在该次研究当中,所选取的100例患者均接受了血液形态学检验,经分析发现对患者进行单独的血液形态学检验,最终对疾病的检出率为45%,但对所选取的100例患者应用血液形态学检验、骨髓活检、免疫分型和细胞遗传学进行联合检验,血液疾病的检出率是100%。两种诊断方法差异有统计学意义(χ2=8.49,P<0.05)。结论在血液病的临床诊断工作当中,血液形态学检验是最为基础也是最为常规的一种临床诊断方法。患者在接受血液形态学检查之后,若是出现诊断不明确的情况,临床工作者则可以根据患者的具体情况给予患者血液形态学检验、骨髓活检、免疫分型和细胞遗传学的联合使用检查,以此来提高患者所患血液病的确诊率,进而为患者疾病后续的治疗提供更加准确、可靠的科学依据。
Objective To study the features of combined examination of blood morphology, bone marrow biopsy, immunophenotype and cytogenetics in hematonosis. Methods 100 cases of hematonosis objects in our hospital from April 2015 to December 2017 were random selected, firstly all patients received the single blood morphological examination, and then received the combined test of multiple diagnosis ways, and the test rate was compared between the two methods. Results Of100 cases, there were 40 cases with thrombocytopenia, 20 cases with chronic granulocytic leukemia, 18 cases with aplastic anemia, 16 cases with chronic lymphocytic leukemia, and 6 cases with myelodysplastic syndrome, and the test rate of single diagnosis and combined diagnosis was respectively 45% and 100%, and the difference was statistically significant（χ2=8.49,P〈0.05）. Conclusion The blood morphological examination is the most basic and routine clinical diagnosis method of hematonosis. After examination, the clinical workers should use the combined examination of blood morphology, bone marrow biopsy, immunophenotype and cytogenetics according to the specific conditions in order to improve the confirmation rate of hematonosis thus providing more accurate and reliable basis for the subsequent treatment.
