摘要 目的在3.0T磁共振中探讨单髋高分辨MRI扫描比传统髋关节MRI扫描显示臼唇软骨损伤的优越性。方法方便选取研究对象88例,均为2017年1—8月该院骨科骨关节病区收治的临床髋部疼痛患者,均行常规髋关节MR扫描和单髋高分辨力MR扫描,根据图像髋臼唇损伤显示是否清晰进行盲评法评分,并进行统计学分析,对单髋高分辨率MRI和传统髋关节扫描获得的臼唇损伤数据差异比较采用Wilcoxon秩和检验,一致性比较采用Pearson检验。结果 88例患者经关节镜证实髋臼损伤为65例,单髋高分辨率扫描敏感度为76.92%(50/65)(医师A)、80.00%(52/65)(医师B);传统髋关节扫描敏感度为53.84%(35/65)(医师A)、55.38%(36/65)(医师B)。因此单髋高分辨力MR扫描比常规髋关节MR扫描在显示软骨损伤方面具有优势,差异有统计学意义(Z=0.436,P<0.05),一致性良好(K=0.675,P<0.05)。结论单髋高分辨力MRI扫描能更清楚的显示髋臼盂唇及关节软骨损伤,有助于早期诊断髋关节撞击综合征(FAI)。 Objective The superiority of single-hip high-resolution MRI scan in 3.0 T magnetic resonance imaging compared with conventional hip MRI scans showed that the labral cartilage injury was shown. Methods 88 subjects were selected conveniently. All were clinical hip pain patients who were admitted to orthopaedic joints in the hospital from January to August 2017. Conventional hip MR scans and single hip high-resolution MR scans were performed. The acetabular lip injury of the image was clearly scored by blind evaluation and statistical analysis was performed. The Wilcoxon rank sum test was used to compare the differences in data of the labrum injury scores obtained from single hip high-resolution MRI and traditional hip joint scans. Consistency comparison was used by Pearson test. Results 88 patients had arthroscopically confirmed acetabular injury in 65 patients. The high-resolution scan sensitivity of single hip was 76.92%(50/65)(Physician A),80.00%(52/65)(Physician B); traditional hip joints of scan sensitivity was 53.84%(35/65)(Physician A), 55.38%(36/65)(Physician B). Therefore, the single-hip high-resolution MR scan was superior to the conventional hip joint MR scan in displaying cartilage injury. The difference was statistically significant(Z=0.436, P〈0.05), and consistency was good(K=0.675,P〈0.05). Conclusion Single-hip high-resolution MRI scans can more clearly show acetabular labrum and articular cartilage injuries, and can help early diagnosis of hip impact syndrome(FAI).
