摘要
目的探讨八股Orthocord线微创治疗肩锁关节脱位伴锁骨远端骨折的临床疗效。方法回顾性分析2016年1月~2017年4月采用八股Orthocord线微创治疗的12例肩锁关节脱位(RockwoodⅢ型)伴锁骨远端骨折(NeerⅡB型)患者的临床资料。结果本组手术时间平均43.5(40~55)min,术中出血量平均32.5(20~50)ml;住院时间平均6.1(5~10)d。11例切口甲级愈合,另1例术后第4 d出现切口脂肪液化,予对症处理后切口愈合。所有患者随访12~15个月,未见肩锁关节再脱位及喙突或锁骨骨折。按Karlsson标准评定疗效:术后3个月为优10例,良2例,术后12个月优12例;按Neer评分标准评定:优10例,良2例,优良率为100%。结论采用八股Orthocord线微创治疗肩锁关节脱位伴锁骨远端骨折具有创伤小、操作简单、固定牢靠、并发症少、无需二次手术取出内固定物等优点,值得临床推广。
Objective To discuss the clinical effect of minimally invasive treatment of eight Orthocord sutures for acromioclavicular dislocation combined with distal clavicle fracture. Method The clinical data of12 cases of acromioclavicular joint dislocation( Rockwood Ⅲ) with distal clavicle fracture( Neer Ⅱ B) performed minimally invasive surgery of eight Orthocord sutures admitted from January 2016 to April 2017 was analyzed retrospectively. Results The operation duration was 40 to 55 minutes with an average of 43. 5 minutes,the intra-operative blood loss was 20 to 50 ml with an average of 32. 5 ml,and the hospitalization time was 5 to10 days with an average of 6. 1 days. The wound healing was well in 11 cases,but appeared incision fat liquefaction in 1 case. All 12 patients were followed up for 12 to 15 months. No acromioclavicular joint dislocation and condylar or clavicle fractures were found again. According to Karlsson standard,the outcomes revealed excellent in 10 cases and good in 2 cases at 3 months after the operation,and excellent in all 12 cases at12 months after the surgery. The shoulder joint function showed excellent in 10 cases and good in 2 cases according to Neer standard,and the fineness rate was 100%. Conclusion Minimally invasive treatment with eight Orthocord sutures can achieve reliable effect for acromioclavicular joint dislocation combined with distal clavicular fracture with the advantages of less trauma,simple operating,firm fixation,few complications and no reoperation of remove the internal fixation,and it is worthy of clinical generalization.
