体外冲击波治疗腰椎间盘突出症的临床疗效分析 被引量:13

摘要 目的观察体外冲击波治疗腰椎间盘突出症的临床疗效,采用表面肌电图分析患者腰部肌肉生物力学特性。方法选取符合纳入标准的患者38例,按随机数字表法随机分为治疗组(19例)和对照组(19例),对照组采用常规治疗,治疗组则在此基础上采用体外冲击波椎旁痛点治疗,1周2次。观察患者治疗前后数字疼痛量表(NRS)评分、Oswestry功能障碍指数(ODI),同时利用表面肌电图收集患者治疗前后双侧椎旁肌表面肌电信号,包括平均肌电波幅(AEMG)、平均功率频率(MPF)和中位频率的斜率(MFs)。结果治疗2周后NRS、ODI评分减低,AEMG和MPF增加,MFs绝对值降低,与组内治疗前比较差异均有统计学意义(P<0.05),NRS、ODI、MFs、MPF第1周结束后治疗组和对照组差异无统计学意义(P>0.05),而第2周结束后治疗组的NRS、ODI、AEMG及MFs与对照组间差异有统计学意义(P<0.05)。结论体外冲击波治疗能显著缓解LDH患者腰部疼痛、改善腰椎功能评分以及腰部肌肉的电生理状况,提高肌肉的收缩功能。 Objective To investigate the effect of extracorporeal shock wave in the treatment of lumber disc herniation(LDH), and surface electromyography was used to analyze biomechanical characteristic of patients' lumber muscle to make a comprehensive evaluation on its efficacy. Methods Thirty-eight patients were randomly divided into experimental group(n=19) and control group(n=19). The control group was given conventional treatment. They experimental group was treated with extracorporeal shock wave,twice a week for 2 weeks. The numerical rating scale(NRS),and ODI score before and after treatment were observed. The surface electromyography was applied to test the surface electromyography signals. AEMG, MFs and MPF were calculated before and after treatment. Results After treatment,NRS and ODI score were significantly reduced, AEMG and MPF were obviously increased,and the absolute value of MFs was decreased, compared with before treatment, the differences were significant (all P〈0.05). After treatment for 1 week, the NRS, ODI, MFs, MPF between two groups had no significant differences(P〉0.05). After treatment for 2 weeks, the NRS, ODI, AEMG and MFs between two groups had significant differences(P〈0.05). Conclusion The extracorporeal shock wave can significantly relieve lumbar pain, muscle strength, muscle tone and muscle fatigue,leading to the recovery of biomechanical characteristic.
