摘要
目的通过建立院内预防静脉血栓栓塞症(venous thromboembolism,VTE)的防治管理体系,在医院管理部门严格监管下,由医护人员共同配合,从对入院患者的风险评估、健康宣教到物理及药物预防,逐步完善防治流程与体系的建立,从而有效降低VTE的发生率,进一步保障医疗安全、提高医疗质量。方法以黑龙江某三甲医院为研究对象,通过文献阅读、专家访谈及头脑风暴等方法,构建院内VTE防治管理体系,以高危试点科室为基础,逐步在全院开展VTE防治与管理工作,并对初步的实施效果进行研究与评价。结果医护人员对VTE的防治意识明显提高,确诊率与治疗率提高,且并未造成平均住院日增加等影响。沟通、宣教及合理用药等防治工作更加规范,防治流程不断完善,总体取得了初步的成效。结论院内VTE综合预防与管理项目的实施能够有效提高VTE防治的科学化与规范化,对维护医疗安全、减少医疗纠纷具有一定的现实意义。
Objective A management system for the prevention of venous thromboembolism （VTE） was estab-lished. Under the strict supervision of the hospital management department and the cooperation of the medical and nursing staff, from risk assessment, health education to physical and drug prevention, prevention and management system of VTE was gradually improved in order to effectively reduce the incidence of VTE, to further safeguard medi-cal safety, and to improve medical quality. Methods Taking a third class hospital in Heilongjiang Province as the re-search object. Literature reading, expert interviews and brainstorming were used. Building the management system of VTE prevention and control in hospital, based on high-risk pilot department, the prevention and management of VTE have been carried out step by step in the whole hospital, and the preliminary effect of the implementation has been studied and evaluated. Results Medical staff＇s consciousness of prevention and treatment of VTE was improved obviously, and diagnosis rate and treatment rate also improved. And average number of hospital days did not in- crease. Communication, education and rational use of drugs and other prevention and treatment work was more stan-dardized. The process of prevention and control had been continuously improved, and preliminary results had been achieved. Conclusion The implementation of the comprehensive prevention and management project of the VTE in the hospital can effectively improve the scientific and standardization of the prevention and control of VTE. It has certain practical significance to maintain medical safety and reduce medical disputes.
