检测乙肝患者Th1细胞因子、T淋巴细胞亚群、特异性细胞毒T淋巴细胞表达的临床意义 被引量:4

摘要 目的探讨急慢性乙肝患者血清Th1细胞因子、T淋巴细胞亚群、特异性细胞毒T淋巴细胞水平的变化以及意义。方法选择我院在2013年10月至2016年10月期间收治的20例急性乙型肝炎患者为急性乙肝组,20例慢性乙型肝炎活动期患者为慢性乙肝组,体检中心20例健康者为正常对照组,测定三组的T淋巴细胞相关细胞因子、T淋巴细胞亚群及特异性细胞毒T淋巴细胞的表达。结果急性乙肝组的IL-2、TNF-α、IFN-γ水平均显著高于正常对照组及慢性乙肝组(P<0.05),慢性乙组肝的IL-2、TNF-α、IFN-γ水平显著低于正常对照组(P<0.05)。急性乙肝组的CD3+、CD4+、CD8+、CD4/CD8水平均显著高于慢性乙肝组(P<0.05),慢性乙肝组的CD3+、CD4+、CD8+、CD4/CD8水平显著低于正常对照组(P<0.05)。急性乙肝组的特异性细胞毒T淋巴细胞表达显著高于正常对照组及慢性乙肝组(P<0.05),慢性乙肝组的特异性细胞毒T淋巴细胞表达显著低于正常对照组(P<0.05)。结论检测乙肝患者的Th1细胞因子、T淋巴细胞亚群及特异性细胞毒T淋巴细胞的表达,可监测疾病发展、病毒复制状态、转归,对指导抗病毒治疗和评价疗效有重要意义。 Objective To investigate the changes and significance of Th1 cytokines, T-lymphocyte subsets and specific cytotoxic T lymphocytes in the serum of patients with acute and chronic hepatitis B. Methods 20 cases of patients with acute hepatitis B admitted to our hospital from October 2013 to October 2016 were selected as the acute hepatitis B group; 20 cases of patients with chronic hepatitis B at active period were selected as the chronic hepatitis B group; 20 healthy people from the physical examination center were selected as the normal control group. The expressions of T lymphocyte related cytokines, T-lymphocyte subsets and specific cytotoxic T lymphocyte in the three groups were determined. Results The levels of IL-2, TNF-α and IFN-γ in the acute hepatitis B group were significantly higher than those in the normal control group and the chronic hepatitis B group(P〈0.05); The levels of IL-2, TNF-α and IFN-γ in the chronic hepatitis B group were significantly lower than those of the normal control group(P〈0.05). The levels of CD3+, CD4+, CD8+and CD4/CD8 in the acute hepatitis B group were significantly higher than those in the chronic hepatitis B group(P〈0.05); The levels of CD3+, CD4+, CD8+and CD4/CD8 in the chronic hepatitis B group were significantly lower than those in the normal control group(P〈0.05). The levels of specific cytotoxic T lymphocyte in the acute hepatitis B group were significantly higher than those in the normal control group and the chronic hepatitis B group(P〈0.05); The levels of specific cytotoxic T lymphocyte in the chronic hepatitis B group were significantly lower than those in the normal control group(P〈0.05). Conclusions The detection of expressions of Th1 cytokines, T-lymphocyte subsets and specific cytotoxic T lymphocytes in patients with hepatitis B can monitor the disease development, viral replication status and outcome, which has important significance for guiding antiviral therapy and evaluating efficacy.
