摘要
目的探究同期颅骨修补术联合脑室-腹腔分流术治疗重型颅脑外伤的效果。方法选取2016年8月—2017年11月该院收治的70例重型颅脑外伤患者为研究对象,其中先进行脑室-腹腔分流术再于8~12周后进行颅骨修补术的35例患者为A组,同期进行颅骨修补术联合脑室-腹腔分流术的35例患者为B组,观察和分析两组GCS评分、患者预后、手术相关并发症等治疗效果。结果比较两组患者术后的GCS评分,B组明显高于A组,差异有统计学意义(χ~2=3.540,P<0.05);比较两组患者的预后程度,B组的良好例数明显高于A组,差异均有统计学意义(χ~2=4.884,P<0.05)。A组患者的手术相关并发症例数为6例(17.14%),B组患者的手术并发症例数为1例(2.86%),差异有统计学意义(χ~2=3.968,P<0.05)。结论同期颅骨修补术联合脑室-腹腔分流术治疗重型颅脑外伤能够明显提高患者的GCS评分和恢复水平,并且可以减少手术并发症的发生率。
Objective To study the effect of cranioplasty for the same period combined with ventriculo-peritoneal shunt in treatment of severe cerebral injury. Methods 70 cases of patients with severe cerebral injury admitted and treated in our hospital from August 2016 to November 2017 were selected and divided into two groups with 35 cases in each, the group A used the ventriculo-peritoneal shunt firstly, and after 8-12 weeks, cranioplasty, and the group B were treated with cranioplasty for the same period combined with ventriculo-peritoneal shunt, and the GCS score, prognosis of patients, and operation related complications of the two groups were observed and analyzed. Results The postoperative GCS score in the group B was obviously higher than that in the group A, and the difference was statistically significant,（χ2=3.540,P〈0.05）; and the prognosis degree good rate in the group B was obviously higher than that in the group A,and the difference was statistically significant（χ2=4.884,P〈0.05）, and the operation related complications cases in the group A and in the group B was respectively 6 cases（17.14%） and 1 cases（2.86%）, and the differences were statistically significant,（χ2=3.968,P〈0.05）. Conclusion The cranioplasty for the same period combined with ventriculo-peritoneal shunt in treatment of severe cerebral injury can obviously improve the GCS score and recovery level, and reduce the incidence rate of operation complications.
