摘要
目的探讨低分子肝素联合强的松治疗肾病综合征的临床疗效及对肾功能、凝血系统的影响。方法 2016年5月—2017年6月间对60例在该院接受治疗的肾病综合征患者进行资料回顾,将其按照随机原则分为两组,对照组中30例患者给予强的松治疗,观察组30患者给予口服强的松联合皮下注射低分子肝素,观察两组患者的疗效、肾功能指标以及凝血系统变化。结果观察组患者总有效率为93.33%明显优于对照组差异有统计学意义(χ~2=6.67,P<0.05);治疗后观察组患者24 h尿蛋白定量、Scr、BUN均明显低于对照组,Cer高于对照组,组间比较,差异有统计学意义(t=6.29、4.29、3.68、4.09,P<0.05);经过治疗后凝血指标观察组患者PT为(14.02±1.25),APTT(31.26±5.94),与对照组相比,差异无统计学意义(t=0.01、0.02,P>0.05)。结论为肾病综合征患者使用低分子肝素联合强的松进行治疗效果显著,肾功能以及凝血系统均得到明显改善,值得临床大力推广。
Objective To study the clinical curative effect of low molecular heparin combined with prednisone in treatment of nephrotic syndrome and effect on the renal function and blood coagulation system. Methods 60 cases of patients with nephrotic syndrome admitted and treated in our hospital were revied, and randomly divided into two groups with 30 cases in each, the control group were treated with prednisone, while the observation group were treated with low molecular heparin combined with prednisone, and the curative effect, renal function and changes of coagulation system of the two groups were observed. Results The total effective rate in the observation group was 93.33%, which was obviously better than that in the control group（χ2=6.67,P〈0.05）, after treatment, the 24 h urine protein quantitation, Scr, BUN in the observation group were obviously better than those in the control group, Cer was higher than that in the control group, and the difference was obvious（t=6.29,4.29,3.68,4.09,P〈0.05）, after treatment, the differences in the coagulation indexes such as PT and APTT between the observation group and the control group were not statistically significant（t=0.01,0.02,P〉0.05）. Conclusion The effect of low molecular heparin combined with prednisone in treatment of nephrotic syndrome is obvious, and the renal function and coagulation system can be obviously improved, and it is worth clinical great promotion.
