摘要
目的探析退变性脊柱侧凸后路手术治疗的中期效果。方法方便选取2012年8月—2016年8月该院收治的40例退行性脊柱侧后凸患者作为研究对象,所有患者均在该院行后路手术,对患者的病历资料进行回顾性分析,并对患者进行为期1年的随访,分析患者的手术效果、术后6个月与术后12个月的影像学检查结果、关节疼痛情况,评价其中期疗效。结果患者术后6个月与1年的腰痛评分分别为(2.6±1.5)分、(1.6±1.1)分,腿痛评分分别为(1.8±1.0)分、(1.5±0.4)分,与治疗前VAS评分对比差异有统计学意义(P<0.05);患者术后6个月与术后12个月的各项影响学检查指标与治疗前对比差异有统计学意义(P<0.05),且冠状面平衡、矢状面平衡术后12个月的影像学检查结果与术后6个月的影像学检查结果对比差异有统计学意义(P<0.05);患者术后6、12个月ODI(功能障碍)指数与治疗前对比差异有统计学意义(P<0.05);并发症发生率为20%(8例),所有患者伤口愈合良好,未见脊柱侧凸进行性加重。结论后路手术治疗退变性脊柱侧凸效果良好,可有效恢复患者的关节功能,纠正关节畸形,并有效缓解疼痛,提高了患者的术后生活质量,但在术后要注意加强并发症的防治,以减少并发症发生率。
Objective To observe the intermediate-term effect of posterior surgery for degenerative scoliosis. Methods 40 cases of patients with degenerative scoliosis admitted and treated in our hospital from August 2012 to August 2016 were convenient selected as the research objects, and all patients underwent the posterior surgery, and the medical records data were reviewed, and the patients were followed up for 1 year, and the operation effect, imaging examination results and joint pains in 6 months and 1 year after surgery were analyzed, and the intermediate-term effect was evaluated. Results The lumbago and skelalgia of patients in 6 months and 12 months after surgery were respectively（2.6±1.5）points,（1.6±1.1）points and（1.8 ±1.0）points,（1.5 ±0.4）points, and the difference in the VAS sore between the two groups before treatment was statistically significant（P〈0.05）, and the differences in various examination indicators in 6 months and 12 months after surgery between the two groups before surgery were statistically significant（P〈0.05）, and the differences in the imaging examination results in 12 months and 6 months after the coronal balance and sagittal balance surgery between the two groups were statistically significant（P〈0.05）, and the difference in the ODI in 6 months and 12 months after surgery before treatment between the two groups was statistically significant（P〈0.05）, and the incidence rate of complications was 20%（8 cases）, and the wound of all patients was good, without the progressive aggravation of rhachioscoliosis. Conclusion The effect of posterior surgery in treatment of degenerative scoliosis is good, which can effectively recover the joint function of patients, correct the joint deformity, effectively relieve the pains, and improve the quality of life after surgery, and we should pay attention to the prevention and treatment of complications after surgery to reduce the incidence rate of complications.
