不同内径导管对高血压脑室出血患者侧脑室外引流效果的影响 被引量:1

摘要 目的探讨高血压脑室出血患者采用不同内径的引流管行侧脑室引流术对疗效的影响。方法回顾性分析2011年1月—2017年1月于我院就诊的43例高血压脑室出血行侧脑室引流术患者的临床资料,按术中使用引流管直径大小分为粗引流管组(A组,23例)和细引流管组(B组,20例),比较2组患者的疗效、术后6个月的生活能力和并发症情况。结果A组引流时间明显短于B组(P〈0.05),且引流中未发生导管堵塞,B组发生4例(11次)导管堵塞。A组的治疗总有效率明显高于B组(P〈0.05),但2组病死率差异无统计学意义(P〉0.05)。2组患者术后6个月日常生活能力量表(ADL)评分差异无统计学意义(P〉0.05);A组患者术后颅内感染并发症发生率明显低于B组(P〈0.05),2组其他并发症发生率差异无统计学意义(P〉0.05)。结论高血压脑室出血患者行侧脑室外引流术,采用粗引流管能有效降低导管发生堵塞的风险,减少导管留置时间,改善治疗效果,并能减少颅内感染风险。 Objective To investigate the effects of drainage tube in different internal diameters on ventricle drainage in patients with hypertensive intraventrieular hemorrhage. Methods Retrospective analysis was performed using clinical data from 43 patients with hypertensive intraventricular hemorrhage who underwent ventricle drainage from January 2011 to January 2017. According to the diameter of the drainage tubes used, they were divided into a large catheter group ( Group A, n = 23 ) and a small catheter group ( Group B, n = 20). Then, the two groups were compared for the efficacy, prognosis and complications six months after operation. Results Group A showed a remarkably shorter drainage time than Group B (P 〈0. 05 ). No catheter blockage in the drainage occurred in Group A, in contrast with four cases (11 times) in Group B. The total effective rate of Group A was significantly higher than that of Group B (P 〈0.05) , but no significant difference was found as to mortality between the two groups ( P 〉 0. 05 ). There was no significant difference in ADL score between the two groups, six months, operation ( P 〉 0.05 ). The incidence of complications of intracranial in- fection in the Group A was significantly lower than that in Group B ( P 〈 0.05 ) after operation, and there was no significant difference in the incidence of other complications between the two groups ( P 〉 0.05 ). Conclusions During ventricle drainage for patients with hypertensive intraventrieular hemorrhage, the use of catheters in large diameters can effectively reduce the risk of catheter blockage, reduce catheter indwelling time, improve the treatment effect, and reduce the risk of intracranial infection.
