摘要
目的：探讨心理咨询在高龄二胎不孕症患者辅助生殖技术治疗中的应用效果。方法;以2015年3月-2016年12月在院实施辅助生殖技术（ART）并接受心理咨询治疗的40例高龄二胎不孕症患者（研究组）和同期于我院实施辅助生殖技术治疗的40例高龄二胎不孕症患者（对照组）为对象,运用综合医院焦虑抑郁量表（HADS）评分评估两组治疗前后心理状况,检测治疗前后两组患者激素[卵泡生成激素（FSH）、黄体生成激素（LH）、雌二醇（E2）]水平变化,比较两组临床妊娠率、生化妊娠率及不良妊娠结局发生率。结果：治疗前两组焦虑评分、抑郁评分、FSH、LH、E2相较无明显差异（P〉0.05）;与治疗前相较,治疗后两组焦虑评分、抑郁评分明显降低,FSH、LH、E2明显升高,差异显著（t研究组=-8.683,-10.308,7.936,9.111,8.841,t对照组=-5.239,-8.348,6.360,7.049,8.058;P〈0.001）,并且治疗后研究组的各项指标变化幅度较对照组明显,差异有统计学意义（t=-5.577,-7.621,3.240,3.695,3.389;P〈0.001）;研究组临床妊娠率较对照组明显高,而生化妊娠率及不良妊娠结局发生率较对照组明显低,差异有统计学意义（χ~2=4.528,-7.813;P〈0.05）。结论：心理咨询在高龄二胎不孕症患者辅助生殖技术治疗中有明确应用效果,可有效缓解患者焦虑抑郁情绪、调节性激素水平,且明显提高临床妊娠率并有效降低生化妊娠率,此外还可有效改善患者妊娠结局。
Objective：To explore the application of psychological counseling in the treatment with assisted reproductive technology for high-aged patients with second-child infertility.Methods：A total of 40 high-aged patients with second-child infertility who were given treatment with assisted reproductive technology（ART）and psychological counseling（study group）and 40 elderly patients with second-child infertility who were given treatment with assisted reproduction technology alone（control group）in the hospital from March 2015 to December 2016 were enrolled in the study.The psychological status of two groups were evaluated with the Hospital Anxiety and Depression Scale（HADS）before and after treatment.Changes of levels of hormones[follicle stimulating hormone（FSH）,luteinizing hormone（LH）,estradiol（E2）]in the two groups were detected,and the clinical pregnancy rate,biochemical pregnancy rate and the incidence of adverse pregnancy outcomes were compared between the two groups.Results：There were no significant differences in anxiety score,depression score,FSH,LH and E2 between the two groups before treatment（P 0.05）.After treatment,anxiety scores and depression scores were significantly decreased,levels of FSH,LH and E2 were significantly increased in the two groups（t=-8.683,-10.308,7.936,9.111,8.841,-5.239,-8.348,6.360,7.049,8.058;P〈0.001）,and changes of all indexes in study group were greater than those in the control group（t=-5.577,-7.621,3.240,3.695,3.389;P〈0.001）.The clinical pregnancy rate of the study group was significantly higher than that of the control group,while the biochemical pregnancy rate and the incidence of adverse pregnancy outcomes were significantly lower than those in the control group（χ~2=4.528,7.813;P〈0.05）.Conclusion：The effect of psychological counseling is clear in the treatment with assisted reproductive technology for high-aged patients with second-child infertility.It can effectively alleviate the anxiety and depression,regulate hormone lev
