摘要
输血在临床上是非常常见的支持疗法,特别是在手术患者和贫血患者中应用广泛。常见的血液制品有红细胞悬液,血浆和血小板等。其中,红细胞制品针对不同患者的特殊需要,可以做一些处理,比如去白细胞,洗涤或者辐照后,得到去白红细胞,洗涤红细胞和辐照红细胞。红细胞悬液经过处理后可以减少输血相关的不良反应,比如非溶血性发热反应,输血过敏反应,HLA同种异体免疫反应,输血相关的移植物抗宿主反应等。其次,对于一些特殊的患者,还需要对红细胞制品进行CMV和HbS检测,来确保该类患者输血的安全性。本文对这几种特殊处理后的红细胞制品的临床应用进行综述,希望能对临床合理用血有所帮助,降低患者输血不良反应的发生率。
Blood transfusion is a very common supportive therapy, especially for surgical patients and anemic patients. The blood products used frequently are red blood cells, plasma and platelets. The red blood cell products, in particular, can be customized to meet patient-specific requirements, such as depleting leukocyte, washing or irradiation. Thus, the red blood cell products are leukocyte-reduced/free, washed or irradiated. These treated products are designed to reduce adverse transfusion reactions, such as non-hemolytic fever, HLA allogeneic immune response and blood transfusion-related graft-versushost response. Second, to ensure the safety of some special patients during blood transfusion, CMV and HbS detection are needed. In this paper, the clinical application of these special red blood cell products is reviewed. We aim to provide insights of refined and safer blood use and thus reduce the incidence of adverse blood transfusion in patients.
