摘要
目的:观察超声乳化术并玻璃体切除术联合羟苯磺酸钙对白内障合并增生性糖尿病视网膜病变(PDR)的疗效。方法:PDR合并白内障患者120例151眼,随机分为对照组和观察组。对照组60例77眼采用超声乳化术合并玻璃体切除术治疗,术后给予常规抗菌药、激素及降眼压治疗;观察组60例74眼在对照组的治疗基础上加用羟苯磺酸钙治疗3个月。比较两组患者手术前后视力、荧光素眼底血管造影(FFA)和光学相干断层扫描(OCT)检查结果。结果:术后1个月和3个月,两组患者的矫正视力均较术前有显著提高(P<0.05),黄斑中心凹视网膜厚度显著低于术前(P<0.05);且观察组患者的矫正视力均显著高于对照组(P<0.05),黄斑中心凹视网膜厚度均显著低于对照组(P<0.05)。术后1个月和3个月,观察组FFA检查黄斑区荧光渗漏比例显著低于对照组(P<0.05)。结论:超声乳化术并玻璃体切除术联合羟苯磺酸钙对白内障合并PDR的疗效较单纯手术更好,能有效降低术后黄斑区视网膜厚度,减少黄斑区毛细血管荧光渗透,提升患者术后视力。
Objective: To evaluate the efficacy of phacoemulsification and vitrectomy combined with calcium dobesilate in the treatment of cataract with proliferative diabetic retinopathy( PDR). Methods: Totally 151 eyes of 120 patients and cataract with PDR were randomly divided into the control group and the observation group. Sixty patients( 77 eyes) in the control group were treated with phacoemulsification and vitrectomy,postoperative routine antibiotics,hormones and lowering intraocular pressure treatment. Sixty patients( 74 eyes) in the observation were treated with the same procedure with the control group besides the extra calcium benzenesulfonate treatment. The visual acuity,fluorescein fundus angiography( FFA) and optical coherence tomography( OCT) were recorded and compared between the groups before and after surgery. Results: The visual acuity of the two groups was significantly improved after the treatment( P〈0. 05),and the visual acuity of the observation group was significantly higher than that of the control group after the treatment( P〈0. 05). After the treatment,the retinal thickness in macular fovea of the observation group was significantly lower than that in the control group( P〈0. 05). FFA examination of macular area fluorescence leakage rate observed in the observation group was significantly lower than that in the control group( P〈0. 05). Conclusion: Phacoemulsification and vitrectomy combined with calcium dobesilate have better curative effect on cataract with PDR than phacoemulsification and vitrectomy alone,which can effectively reduce the retinal thickness of macular area,reduce the glaucoma glaucoma penetration in macular area and improve postoperative visual acuity.
