摘要
目的了解重症监护室医院感染的发生特点,为预防重症监护室医院感染提供对策。方法采用目标性监测方法,对我院2015—2016年重症监护室患者进行医院感染发病情况监测,包括医院感染例次率、调整感染发病率、"三管"感染发病率、医院感染部位分布。结果重症监护室2015、2016年调整感染发病率分别为3.21%及3.24%;2015、2016年的合计"三管"千日感染率分别为4.17‰、15.33‰、4.74‰;两年合计医院感染部位构成比排前三位的依次为下呼吸道感染、泌尿系统感染、菌血症。结论重症监护室医院感染发病率较高,是医院感染控制的重点部门,"三管"感染率较高,尤其是呼吸机相关肺炎的感染率较高,下呼吸道感染占重症监护室医院感染的比重较大,应注意对患者下呼吸道感染与呼吸机相关肺炎的预防。
Objective To investigate the characteristics of nosocomial infection of intensive care unit, and provide countermeasures for the nosocomial infection of intensive care unit. Methods The method of target monitoring was implemented in patients in intensive care units of our hospital from 2015 to 2016 to monitor the incidences of nosocomial infections, including the nosocomial infection rates, the incidences of adjusted infections, incidences of ＂three-tube＂ infection and distribution of nosocomial infection sites. Results The incidence of adjusted infections in intensive care units in 2015 and 2016 was 3.21% and 3.24% respectively; and the total infection incidences of ＂three-tube＂ infection for one thousand days in 2015, 2016 was 4.17%, 15.33%and 4.74%respectively. In the two years, the proportion of nosocomial infections in the top three was lower respiratory tract infection, urinary system infection and bacteremia respectively. Conclusion The high incidence of nosocomial infection of intensive care unit is the focus of the nosocomial infection control. The incidence of ＂three-tube＂ infection is higher, especially the infection rate of ventilator-associated pneumonia, lower respiratory tract infection account for a larger proportion in hospital infection of intensive care unit. It should pay attention to the prevention of lower respiratory tract infection and ventilator-associated pneumoniain patients.
