摘要
目的对比分析中青年和老年2型糖尿病患者非高密度脂蛋白胆固醇(non-HDL-C)、动脉硬化指数(AI)。方法选取2017年江苏大学附属武进人民医院内分泌科收治的2型糖尿病患者346例,根据发病年龄分为中青年组138例(发病年龄24~59岁)和老年组208例(发病年龄60~90岁),对比分析两组患者一般资料和血脂指标。结果老年组患者女性比例、高血压发生率高于中青年组,糖化血红蛋白(Hb A1c)低于中青年组(P<0.05);两组患者体质指数(BMI)、冠心病发生率、吸烟率比较,差异无统计学意义(P>0.05)。老年组患者总胆固醇(TC)、三酰甘油(TG)、载脂蛋白B(Apo B)、non-HDL-C、AI低于中青年组(P<0.05);两组患者低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(Apo A1)比较,差异无统计学意义(P>0.05)。亚组分析结果显示,老年组男性患者TC、TG、Apo B、non-HDL-C、AI低于中青年组,而两组男性患者LDL-C、HDL-C、Apo A1比较,差异无统计学意义(P>0.05);两组女性患者TC、TG、LDL-C、HDL-C、Apo A1、Apo B、non-HDL-C、AI比较,差异无统计学意义(P>0.05)。结论中青年2型糖尿病患者尤其是男性患者non-HDL-C、AI高于老年2型糖尿病患者,non-HDL-C、AI能更全面地反映2型糖尿病患者血脂代谢状况,临床应重视non-HDL-C、AI的控制。
Objective To contrastively analyze non-HDL-C and AI between young and middle-aged and aged patients with type 2 diabetes mellitus. Methods A total of 346 patients with type 2 diabetes mellitus were selected in Wujin Peoples' s Hospital Affiliated to Jiangsu University in 2017,and they were divided into young and middle-aged group(with age of onset from 24 to 59 years old,n=138)and aged group(with age of onset from 60 to 90 years old,n=208)according to the age of onset,general information and blood lipids index were compared between the two groups. Results Female proportion and incidence of hypertension in aged group were statistically significantly higher than those in young and middle-aged group,while Hb A1 c in aged group was statistically significantly lower than that in young and middle-aged group(P〈0.05);no statistically significant differences of BMI,incidence of coronary heart disease or smoking rate was found between the two groups(P〉0.05). TC,TG,ApoB,non-HDL-C and AI in aged group were statistically significantly lower than those in young and middle-aged group(P〈0.05),while no statistically significant differences of LDL-C,HDL-C or ApoA1 was found between the two groups(P〉0.05). Subgroup analysis results showed that,TC,TG,ApoB,non-HDL-C and AI in males in aged group were statistically significantly lower than those in young and middle-aged group,while no statistically significant differences of LDL-C,HDL-C or ApoA1 was found in males between the two groups(P〉0.05);no statistically significant differences of TC,TG,LDL-C,HDL-C,ApoA1,ApoB,non-HDL-C or AI was found in females between the two groups(P〉0.05). Conclusion Non-HDL-C and AI in young and middle-aged patients with type 2 diabetes mellitus are significantly higher than those in aged patients with type 2 diabetes mellitus,non-HDL-C and AI can more roundly reflect the blood lipid metabolism status in patients with type 2 diabetes mellitus,thus clinicians should pay more attentions to the control of non-HDL-
