摘要
目的了解滥用一氧化二氮(N_2O)所致的神经系统损害的临床症状与体征。方法本院收治1例滥用N_2O导致脊髓病变的患者的临床特点。结果长期接触N_2O可通过干扰维生素B_(12)的生物利用而引起神经毒性作用,导致痉挛性截瘫、深浅感觉障碍、二便障碍等脊髓病表现,大剂量维生素治疗后好转。结论滥用N_2O可造成脊髓亚急性联合变性(SACD)的临床症状与体征,通过该病例,提醒临床医生对此病因加以关注,避免误诊。
Objective To analyze the causes and manifestations of nervous system damage caused by the abuse of nitrous oxide （ N2O ） . Method Summarize one case＇s clinical features of N2O abuse admitted in our hospital recently lead to spinal cord lesions. Results Long term exposure to N2O can interfere with the bioavailability of vitamin B12 caused neurotoxicity. The main manifestations of this case include spastic paraplegia, sensation disorder, urinary retention and difficulty of defecation. All manifestations have relieved after the treatment of large doses of vitamins. Conclusion The abuse of N2O can lead to subacute combined degeneration （ SACD ） of the spinal cord. Through the clinical characteristics of the disease and related literature review, we strengthen the clinicians＇ understanding of the disease.
