摘要
目的探讨血清胰岛素样生长因子-1(insulin-like growth factor 1,IGF-1)水平与2型糖尿病肾病(type 2 diabetic nephropathy,T2DN)患者早期肾损伤标志物水平的关系。方法选取本院2016年1月至2018年2月期间收治的T2DN患者400例为研究对象,根据尿微量蛋白排泄率(UAER)将纳入研究的患者分为两组:单纯糖尿病组(SDM组)和糖尿病肾病组(DN组)。比较两组IGF-1水平、清晨空腹血糖(FPG)和餐后2h血糖(2hPG)、UAER、血清胱抑素C(Cys-C)和尿视黄醇结合蛋白(RBP)水平。结果两组FPG、2hPG比较,差异无统计学意义(P>0.05),SDM组UAER、Cys-C、RBP和IGF-1水平分别为(12.93±1.28)μg/min、2.32(1.93~3.27)mg/L、(3.71±0.53)mg/L和147.43(109.32~174.27)ng/mL,显著低于DN组(P<0.05);IGF-1>200ng/mL组的UAER、Cys-C和RBP水平分别为(141.72±31.82)μg/min、4.35(4.02~4.93)mg/L和(6.46±1.07)mg/L,显著高于IGF-1<200ng/mL组(P<0.05);经相关性分析,IGF-1水平与UAER、Cys-C和RBP均呈正相关(r=0.293,r=0.388,r=0.402,P均<0.05)。结论 IGF-1水平与T2DN患者早期肾损伤标志物呈正相关,可能是预测T2DN早期肾损伤的新指标。
Objective To explore the relationship between serum insulin- like growth factor- 1 ( IGF- 1 ) level and early renal injury markers' levels in patients with type 2 diabetic nephropathy(T2DN). Methods 400 cases of T2DN patients treated in our hospital from January 2016 to February 2018 were selected for the study and divided into two groups according to the urinary albumin excretion rate (UAER) :the simple diabetes mellitus group ( the SDM group ) and the diabetic nephropathy group ( the DN group). The levels of IGF- 1, morning fasting blood glucose (FPG) and postprandial 2h blood glucose ( 2hPG), UAER, serum Cystatin C ( Cys- C) and urinary retinol binding protein (RBP)were compared between the two groups. Results There was no significant difference in the levels of FPG and 2hPG between the two groups ( P 〉 0.05 ). The levels of UAER,Cys-C,RBP and IGF-1 were (12. 93 ±1. 28 ) μg/min, 2. 32 (1. 93 - 3. 27 ) mg/L, (3. 71 ± 0.53 ) mg/L and 147.43 ( 109.32,174.27 ) ng/mL in the SDM group respectively, which were significantly lower than those in the DN group(P 〈 0.05 ). The levels of UAER,Cys-C and RBP were( 141.72 ± 31.82) μg/min,4.35(4.02,4.93 )mg/L and(6.46 ± 1.07)mg/L respectively in the IGF-1 〉 200ng/mLgroup, which were significantly higher than those of the IGF- 1 〈 200ng/mL group ( P 〈 0.05 ). By conducting the correlation analysis, we found that the level of IGF-1 was positively correlated with UAER, Cys-C and RBP ( r = 0. 293, r = 0. 388, r = 0.402, all P 〈 0.05 ). Conclusion The IGF- 1 level is positively correlated with early renal injury markers in the T2DN patients and may be used as a new index for predicting the early renal injury in T2DN.
