摘要
[目的]观察柴贝止痫汤联合西药治疗难治性癫痫复杂部分性发作疗效。[方法]使用随机平行对照方法,将60例住院患者按就诊顺序号随机分两组。对照组30例丙戊酸钠,0.5g/次,2次/d;苯妥英钠,0.1g/次,3次/d;卡马西平,0.1g/次,2次/d。治疗组30例柴贝止痫汤(柴胡12g,浙贝母、石菖蒲各9g,地龙6g,天麻15g,半夏^(法)9g,牡蛎30g;清热解毒加黄芩12g,黄连6g),1剂/d,水煎300~400m L,早晚口服;西药治疗同对照组。连续治疗4周为1疗程。观测临床症状、中医症状积分量表、蒙特利尔认知评估(Mo CA)、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组控制12例,明显改善8例,中度改善5例,轻度改善4例,无改变1例,恶化0例,总有效率83.33%;对照组控制8例,明显改善4例,中度改善6例,轻度改善7例,无改变3例,恶化2例,总有效率60.00%;治疗组疗效优于对照组(P<0.05)。症状积分、Mo CA评分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]柴贝止痫汤联合西药治疗难治性癫痫复杂部分性发作,疗效满意,无严重不良反应,随治疗时间延长,中医症状改善更加明显,值得推广。
[Objective] To observe the efficacy of Chaibei Zhixian decoction combined with western medicine in the treatment of intractable epilepsy complicated partial seizures. [Methods] Using random parallel control method,60 hospitalized patients were randomly divided into two groups according to the order number. Control group： 30 cases of valproate,0.5 g/times,2 times/d; phenytoin sodium,0.1 g/times,3 times/d; C Masi Bing,0.1 g/times,2 times/d. In the treatment group,30 cases of chaibezhi epilepsy decoction（Chaihu 12 g,Zhebeimu,Shicangpu each 9 g,Dilong 6 g,Tianma 15 g,Fabanxia 9 g,Muli 30 g;Qingre Jiedu ad Huangqin 12 g,Huanglian 6 g）,1 doses of/d,water decoction and 1000 mL,were taken orally in the morning and evening; western medicine was treated in the same control group. Continuous treatment for 4 weeks was the 1 course of treatment. Observation of clinical symptoms,TCM symptom score scale,Montreal cognitive assessment（MoCA）,adverse reactions. The 3 course of treatment was continuously treated to determine the curative effect. [Results] the treatment group control 12 cases,obviously improved 8 cases,moderate improvement in 5 cases,mild improvement in 4 cases,no changes in 1 cases,0 cases of deterioration,total effective rate 83.33%,control group control 8 cases,4 cases,moderate improvement 6 cases,mild improvement 7,no change 3 cases,deterioration 2 cases,total effective rate 60%; treatment group effect is superior to the control group（P〈0.05）. Symptom score and MoCA score in the treatment group were better than those in the control group（P〈0.05）. [Conclusion] Chai Bei Zhi Xian decoction combined with western medicine is effective in treating complex partial seizures of intractable epilepsy. There is no serious adverse reaction. It is worth promoting.
