摘要
目的探讨经皮左心耳封堵术预防房颤相关脑卒中护理配合的要点。方法回顾性分析2016年6月至2017年5月在上海长海医院影像医学科导管室行经皮左心耳封堵(left atrial appendage closure,LAAC)治疗的房颤卒中高危患者40例的临床资料,总结其护理配合要点。结果 39例患者手术均在食道超声引导下顺利完成,其中38例患者使用Lacbes左心耳封堵器,1例患者使用ACP左心耳封堵器。术中1例患者发现心包填塞行急诊心脏修补术及封堵器取出,其余患者均顺利完成手术,无护理相关并发症发生。结论介入导管室护士正确掌握经皮左心耳封堵术的手术过程以及可能出现的并发症,掌握手术配合方法,术前做好患者的心理护理以及各项术前准备,术中与医生密切配合,对于患者手术成功有着重要意义。
Objective To investigate the main points of nursing coordination of percutaneous left atrial appendage closure(LAAC)to prevent atrial fibrillation related stroke.Methods Clinical data of 40 atrial fibrillation cases at high risk of stroke for attempted percutaneous LAAC in the catheter room of radiology departement of Shanghai Changhai Hospital from June 2016 to May 2017 were retrospectively studied.The nursing points were summarized.Results Percutaneous LAAC was successfully performed on 39 patients under transesophageal echocardiography.Among them,38 cases were implanted with Lacbes LAAC occluder,and 1 case with ACP LAAC occluder.Cardiac tamponade occurred in 1 case of operation and emergency heart repair surgery was performed to take out occluder.All the other patients had not any nursing related complications.Conclusion Nurses working in the catheter room of intervention department should understand the procedure of ring percutaneous LAAC and its potential complications,master methods of operative coordination,provide psychological nursing and preoperative preparation,and cooperate with doctors closely,which is of great importance for the success rate of operation.
