摘要
目的研究瑶族慢性肺源性心脏病患者血浆N端脑钠肽前体(NT-proBNP)水平的临床变化情况。方法选取我院2016年6月至2017年5月明确诊断为慢性肺源性心脏病的200例患者,根据患者的民族分为瑶族组和汉族组各100例。两组患者均给予相同的治疗方法,观察两组患者治疗后的NT-proBNP水平情况,分析不同心功能分级患者治疗前后的NT-proBNP水平情况。结果两组患者治疗前的NT-proBNP水平比较无统计学差异(P>0.05)。治疗前,两组的心功能Ⅱ级患者NT-proBNP水平均明显低于心功能Ⅲ级患者,心功能Ⅲ级患者NT-proBNP水平均明显低于心功能Ⅳ级患者(P均<0.05)。治疗后,两组患者的NT-proBNP水平均有所下降,但瑶族组心功能Ⅱ级、Ⅲ级、Ⅳ级患者的NT-proBNP水平均明显高于汉族组(P<0.05)。结论 NT-proBNP水平可作为检测瑶族慢性肺源性心脏病患者心力衰竭的指标,具有重要作用,为临床治疗提供参考。
Objective To study the clinical change of plasma NT-proBNP levels in patients with chronic pulmonary heart disease（CPHD） in Yao nationality. Methods 200 cases of patients diagnosed as CPHD in our hospital from June 2016 to May 2017 were selected and divided into Yao group and Han group, with 100 cases in each group. Both groups were given the same treatment. The NT-proBNP levels after treatment of the two groups were observed, and the NT-proBNP levels of patients with different cardiac function grades in two groups before and after treatment were analyzed. Results No statistical difference was found in the NT-proBNP levels between the two groups before treatment（P 〉0.05）. Before treatment, the NT-proBNP levels of patients with grade Ⅱ cardiac function in two groups were significantly lower than those of patients with grade Ⅲ cardiac function, and the NT-proBNP levels of patients with grade Ⅲ cardiac function were significantly lower than those of patients with grade Ⅳ cardiac function（all P 〈0.05）. After treatment, the NT-proBNP levels of two groups decreased, but the NT-proBNP levels of patients with grade Ⅱ, Ⅲ and Ⅳ cardiac function of Yao group were significantly higher than those of Han group（P 〈0.05）. Conclusions NT-proBNP level can be used as an detection indicator for heart failure of CPHD patients of Yao nationality, which plays an important role and provides reference for clinical treatment.
