摘要
目的探讨银杏叶提取物(EGb)对老年人髋关节置换术围术期炎性因子、载脂蛋白E(ApoE)及认知功能的影响。方法选择髋关节置换术患者50例,随机分为2组(n=25),对照组(C组)和银杏叶提取物组(E组),在腰硬联合麻醉下完成手术。患者在术前2h(T_0)、术后1天(T_1)及3天(T_2)抽取静脉血,E组抽血后静滴金纳多注射液,采用ELISA测血清IL-1β、IL-6、TNF-α及ApoE的浓度;同时于术前1天、术后1天、3天使用蒙特利尔认知量表(MoCA)测评认知功能。结果与C组比较,E组T_1,时点IL-6、TNF-α及ApoE浓度降低,T_2、时点IL-6、及ApoE浓度降低(P<0.05);E组术后1天,MoCA评分升高(P<0.05)。结论 EGb对髋关节置换术后老年患者认知功能的改善,与抑制围术期炎性反应有关。
Objective To investigate the effect of Ginkgo biloba extract（EGb）on Perioperative periodinflammatory factors,apolipoprotein E（ApoE）and cognitive function in elderly patients undergoing hip replacement surgery.Methods The patients（65-80 years old）undergoing hip replacement surgery under combined Spinal-epidural Anesthesia,were randomly assigned into 2 groups（n=25 each）,control group（group C）and EGb group（group E）.At 2 hbefore surgery,on 1 day and 3 days after surgery.venous blood samples were collected for determination of IL-1β,IL-6,TNF-αand ApoE concentrati by enzyme-linked immunosorbent assay,In group E,EGb injection was infused intravenously after collecting blood samples.The patient＇s cognitive function was assessed using Montreal Cognitive Assessment（MoCA）on 1 day before surgery,on 1 day and 3 days after surgery.Results Compared with group C,in group E the concentrations of IL-6,TNF-αand ApoE were decreased at day 1 after surgery（P〈0.05）;and the concentrations of IL-6 and ApoE were decreased at T2（P〈0.05）.Compared with group C,in group E MoCA scores were increased on day 1 after surgery（P〈0.05）;compared with preoperative,MoCA score were decreased in group C（P〈0.05）.Conclusion EGb can improve the cognitive function following hip replacement in elderly patients,which is related to inhibiting inflammatory reaction in perioperation period.
