摘要
【目的】探讨百令胶囊联合孟鲁司特钠对慢性阻塞性肺疾病(COPD)患者呼吸功能和免疫功能的影响。【方法】将60例COPD患者随机分为观察组和对照组,每组各30例。对照组给予孟鲁司特钠治疗,观察组给予百令胶囊联合孟鲁司特钠治疗。比较2组临床疗效,评估治疗前和治疗8周后呼吸功能[1s用力呼气容积/用力肺活量(FEV1/FVC)、1秒用力呼气容积占预计值百分比(FEV1%)、每分钟最大通气量占预计值百分比(MVV%)]、免疫功能(T淋巴细胞亚群)、疾病相关血清细胞因子[血清转化生长因子-β1(TGF-β1)、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-9(MMP-9)]的变化情况。【结果】(1)治疗8周后,观察组总有效率为93.33%,对照组为73.33%,观察组的总有效率及总体疗效均优于对照组,差异均有统计学意义(P<0.05或P<0.01)。(2)治疗后,2组患者呼吸功能(FEV1/FVC、FEV1%、MVV%)、T淋巴细胞亚群(CD3+、CD4+、CD4+/CD8+)水平均较治疗前提高(P<0.05或P<0.01),疾病相关血清细胞因子TGF-β1、TNF-α、MMP-9水平均较治疗前下降(P<0.01),且观察组对各项指标的改善幅度均大于对照组,差异均有统计学意义(P<0.05)。【结论】百令胶囊联合孟鲁司特钠能提高COPD患者临床疗效,改善其免疫功能和呼吸功能。
Objective To investigate the effects of Bailing Capsules combined with Montelukast Sodium onrespiratory function and immune function of patients with chronic obstructive pulmonary disease（COPD）. Methods Sixty cases of COPD patients were divided into observation group（N = 30）and control group（N = 30）accordingto the random number table. The two groups were treated with Montelukast Sodium,and the observation group wasadditionally treated with Bailing Capsules. The clinical curative effect was compared in two groups, and therespiratory functions [forced expiratory volume in one second/forced vital capacity（FEV1/FVC）,the percentage offorced expiratory volume in one second to the predicted value（FEV1%）,the percentage of maximal ventilationvolume per minute to the predicted value（MVV%）], immune function（T lymphocyte subsets）and disease-related serum cytokines [serum transforming growth factor β1（TGF-β1）,tumor necrosis factor α（TNF-α）,matrix metalloproteinase 9（MMP-9）] were evaluated before treatment and after treatment for 8 weeks. Results（1）After treatment for 8 weeks,the total effective rate in the observation group was 93.33%,and that in the controlgroup was 73.33%. The efficacy in the observation group was better than that in the control group,the differencebeing significant（P〈0.05 of P〈0.01）.（2）After treatment,the levels of respiratory function（FEV1/FVC,FEV1%,MVV%）and T lymphocyte subsets（CD3＋,CD4＋,CD4＋/CD8＋）in the two groups were higher thanthose before treatment（P〈0.05 or P〈0.01）,and the levels of TGF-β1,TNF-α and MMP-9 in the two groupswere lower than those before treatment（P〈0.01）,the improvement in the observation group being superior to thatin the control group（P〈0.05）. Conclusion Bailing Capsules combined with Montelukast Sodium can enhance theclinical curative effect,and improve the immune function and respiratory function of COPD patients.
