摘要
目的比较普通腹腔镜与机器人辅助腹腔镜对肾脏错构瘤病人行保留肾单位手术的疗效差别。方法行手术治疗的肾脏错构瘤病人82例,将82例病人分成普通腹腔镜组和机器人手术组,每组各41例,比较两组病人年龄、性别、体质指数、手术时间、热缺血时间。结果普通腹腔镜组的手术时间(机器人术前准备时间不计算在内)、热缺血时间分别是(150.95±43.27)分钟、(23.88±4.34)分钟,机器人手术组分别是(131.88±42.08)分钟、(20.63±6.00)分钟,两组比较差异有统计学意义(P<0.05);两组病人术后进食时间、拔管时间、住院时间、手术前后肌酐差值、术前术后eG FR差值、术前术后血红蛋白差值比较差异均无统计学意义(P>0.05)。结论对肾脏错构瘤病人施行保留肾单位手术时,机器人手术组与普通腹腔镜组比较,可缩短手术时间,减少热缺血时间。
Objective To evaluate the effect of laparoscopic nephron-sparing surgery( LNSS)and robot-assisted laparoscopic nephron-sparing surgery( RALNSS). Mtheods A retrospective review was performed on 82 patients with renal angiomyolipoma. All the patients was divided into two groups( LNSS and RALNSS) and both the two groups included 41 patients respectively. The operation time,warm ischemic time,intestinal recovery time,time of extubation,time of hospitalization,creatinine difference before and after operation,eG FR difference before and after operation,and the difference of hemoglobin in the two groups were compared.. Results The operating time and warm ischemia time between the two groups were statistically significant( P〈0. 05). Compared with LNSS group,both the operating time[( 150. 95 ± 43. 27) min vs( 131. 88 ± 42. 08) min,P = 0. 046]and warm ischemia time[( 23. 88 ± 4. 34)min vs( 20. 63 ± 6. 00) min,P = 0. 006]were less in RALNSS group. When it's concerned about the rest parameters such as the intestinal recovery time,time of extubation,time of hospitalization,creatinine difference before and after operation,eG FR before and after surgery,there were no statistical significance among them( P〉0. 05). Conclusion The RALNSS group showed great advantages than LNSS group on operating time and warm ischemia time. But there was no significance among the rest parameters of the two groups.
