脉搏门控非增强快速MR成像对不同分型主动脉夹层动脉瘤的诊断敏感度对比 被引量:1

摘要 目的 对比脉搏门控非增强快速MR成像对不同分型主动脉夹层动脉瘤的诊断敏感度.方法 采用回顾性研究方法,选择2010年1月至2017年4月在我院进行诊治的24例主动脉夹层动脉瘤患者作为研究对象,术中病理诊断为A型14例、B型10例,记录所有患者的脉搏门控非增强快速MR成像质量和特征,判断鉴别诊断敏感度.结果 A型与B型主动脉夹层动脉瘤患者的快速MR成像图像都能够满足医生的临床诊断要求,冠状动脉开口、瓣膜交界及主动脉窦的图像质量都为2级和1级.脉搏门控非增强快速MR成像鉴别诊断不同分型主动脉夹层动脉瘤的敏感度和特异度分别为100.0%和90.0%;脉搏门控非增强快速MR成像对瓣膜交界、主动脉窦、冠状动脉开口的评价准确度分别为91.7%、87.5%和87.5%.结论 脉搏门控非增强快速MR成像在不同分型主动脉夹层动脉瘤中的诊断图像优良,具有很高的敏感度和可行性,能有效显示形态学信息,从而指导临床鉴别诊断. Objective To compare the diagnostic sensitivity of pulse gated non enhanced fast MR imaging in different types of aortic dissecting aneurysm.Methods From January 2010 to April 2017,24 cases of aortic dissection admitted in our hospital for diagnosis and treatment were selected as the research object and were divided 10 cases of type A and 14 cases of type by the intraoperative pathological diagnosis.The quality and characteristics of pals-gated non enhanced fast MR imaging in all the patient were recorded,and the differential diagnosis sensitivity were determined.Results The pulse-gated non-enhanced fast MR imaging in the patients with type A and type B aortic dissection were all satisfied the clinical diagnosis,and the image quality of valve junction,aortic sinus and coronary artery opening were all grade 2 and grade 1.The pulse gated non enhanced fast MR imaging in the differential diagnosis of sensitivity and specificity for different types of aortic aneurysm were 100% and 90%;and the accuracy evaluation of mitral valve junction,aortic sinus,coronary artery were 91.7%,87.5% and 87.5%.Conclusion Pulse-gated non-enhanced fast MR imaging in different types of aortic dissection are with excellent sensitivity and high feasibility,it can effectively display morphological information,so as to guide the clinical diagnosis.
