摘要
目的观察陈旧性肱骨骨折并骨质疏松症的交锁髓内钉内固定治疗效果。方法随机抽取2016年8月~2017年8月收治的90例陈旧性肱骨骨折并骨质疏松症患者作为研究对象,所有患者均行手术治疗,根据不同术式将其分为对照组与治疗组,每组均为45例,对照组应用锁定钢板治疗,治疗组应用交锁髓内钉内固定治疗,对比两组患者的治疗效果、临床相关指标以及并发症发生情况。结果对照组治疗效果的优良率是84.44%,治疗组是86.67%,治疗组略高于对照组,对比差异不显著(P>0.05)。两组患者的手术时间对比,差异不显著(P>0.05);两组患者的术中出血量与骨折愈合时间对比,治疗组均少(短)于对照组,差异显著(P<0.05)。对照组的并发症发生率是11.11%,治疗组是4.44%,治疗组低于对照组,对比差异显著(P<0.05)。结论陈旧性肱骨骨折并骨质疏松症的交锁髓内钉内固定治疗效果显著,具有术中出血量少、骨折愈合时间短、并发症发生率低等优势,值得临床大力推行。
Objective To observe the effect of internal fixation with interlocking intramedullary nail for old humeral fracture and osteoporosis. Methods Randomly selected from our hospital in August 2016 - 2017 year in August 90 cases of humerus fracture and osteoporosis patients as the research object,all the patients underwent surgical treatment,according to the different operation style will be divided into control group and treatment group,each group had 45 cases,the control group used locking plate treatment,treatment group the application of interlocking intramedullary nail fixation for the treatment of occurrence,compared two groups of patients,clinical indicators and complications. Results The good rate of treatment in the control group was 84. 44%,the treatment group was 86. 67%,the treatment group was slightly higher than the control group,and the contrast difference was not significant（P〈0. 05）. There was no significant difference in operative time between the two groups（P〈0. 05）. The operative blood loss and fracture healing time in the two groups were shorter than those in the control group（P〈0. 05）. The incidence of complications in the control group was 11. 11%,the treatment group was 4. 44%,the treatment group was lower than the control group,the difference was significant（P〈0. 05）. Conclusions Interlocking intramedullary nailing for the treatment of old humeral fractures and osteoporosis is effective,with less bleeding volume,shorter fracture healing time and lower incidence of complications. It is worthy of clinical implementation.
