维持性血液透析患者中血清同型半胱氨酸与心血管疾病相关性研究 被引量:10

Effects of plasma homocysteine on the cardiovascular disease in maintenance hemodialysis patients

摘要 目的探讨血清同型半胱氨酸对维持性血液透析患者心血管系统的影响。方法选择2007—2013年间中国医科大学附属第一医院血液净化门诊维持性血液透析(maintenance hemodialysis,MHD)的患者30例作为研究对象(MHD组),同时选取20名健康体检者作为对照组。比较两组血清同型半胱氨酸、超敏C反应蛋白、颈动脉中膜厚度等指标的差异;将透析患者血压作为分层因素,比较不同组间同型半胱氨酸及颈动脉中膜厚度的差异。结果MHD组同型半胱氨酸水平与超敏C反应蛋白(ultra sensitive C reactive protein,CRP)、颈动脉中膜厚度、室间隔厚度等成正相关。透析患者中高血压亚组同型半胱氨酸的水平升高,颈动脉中膜增厚。结论多数MHD患者存在动脉硬化及左心肥厚,同型半胱氨酸是透析患者左心肥厚的独立危险因素。 Objective To explore the effection of the serum homocysteine on cardiovascular system in patients with maintenance hemodialysis. Methods From 2007 to 2013, 30 patients were selected for maintenance hemodialysis(MHD) in the blood purification outpatient clinic of the First Hospital of China Medical University, and 20 healthy health examiners were selected as control groups. The differences of serum homocysteine, hypersensitive C-reactive protein and carotid middle membrane thickness were compared between the two groups. The blood pressure of dialysis patients was used as a stratification factor to compare the difference of homocysteine and middle membrane thickness of carotid artery between different groups.. Results The homocysteine levels in group MHD were positively correlated with the levels of CRP, the thickness of the carotid artery, and the thickness of the ventricular septum. The level of homocysteine in patients with hypertension was increased, and the thickness of carotid artery was increased. Conclusion Most patients with maintenance hemodialysis, have arterial stiffness and left ventricular hypertrophy, and homocysteine is an independent risk factor for left ventricular hypertrophy in dialysis patients.
