铁调素在PD贫血患者中的表达及与铁代谢的关系 被引量:1

Change and role of hepcidin from pre-treatment to post-treatment of iron supplements in peritoneal dialysis patients with anemia

摘要 目的探讨铁调素(hepcidin)在腹膜透析(PD)贫血患者中的表达及铁剂治疗前后其表达的变化及与铁代谢的关系。方法选取伴有肾性贫血的腹膜透析患者50例,分为:铁缺乏组27例,非铁缺乏组23例,选取体检健康的志愿者30名为正常对照组。随访6个月,用酶联免疫吸附法(ELISA)测定各组中血清铁调素的水平,测定铁指标及其它临床及生化指标,铁缺乏组予常规补铁,观察铁调素及铁指标的变化,观察非铁缺乏病人铁调素及铁指标的变化,分析铁调素与铁指标的相关性。结果腹膜透析贫血病人血清铁调素水平较健康对照组明显增加,分别为(99.7±36.7)ng/ml,(78.4±20.6)ng/ml,差异有统计学意义(P<0.05);铁缺乏患者血清铁调素较非铁缺乏者明显下降,分别为:(69.4±27.8)ng/ml,(112.4±40.2)ng/ml,差异有统计学意义(P<0.01);铁缺乏纠正后铁调素水平较前明显升高,为(106.4±34.3)ng/ml,差异有统计学意义(P<0.01);非铁缺乏病人在随访过程中出现铁缺乏时血清铁调素水平明显下降,分别为(112.4±40.2)ng/ml,(85.7±38.6)ng/ml,差异有统计学意义(均P<0.05),铁调素与血清铁、血清铁蛋白、转铁蛋白饱和度呈正相关(r分别为0.26,0.30,0.24,P值分别为0.003,0.005,0.004)。结论腹膜透析贫血患者血清铁调素水平明显升高,铁调素水平随着铁指标的变化而变化,并参与了腹膜透析病人的铁代谢过程。 Objective To investigate the expression of hepcidin in peritoneal dialysis( PD) patient with anemia and to evaluate the changes and role of hepcidin from pre-treatment to post-treatment of iron supplements. Methods Totally 50 peritoneal dialysis patients who were diagnosed with renal anemia were chosen as observation group,and they were further divided into two groups : iron deficiency group,27 cases; non-iron deficiency group,23 cases. 30 healthy volunteers with normal health checkup results from the Health Checkup Center of our hospital were selected as normal control group. After following up for 6 months,the serum hepcidin levels in patients of every group were detected by enzyme linked immunosorbent assays( ELISA),Indicators of iron and other clinic and biochemistry were also detected. Iron supplements were routinely given to patients in iron deficiency group,the changes of hepcidin and iron-related parameters after iron deficiency correction were observed. For patients in non-iron deficiency group,the changes of hepcidin and iron-related parameters during follow-up were observed. The relations between hepcidin and iron-related parameters were analyzed. Results Serum hepcidin levels in peritoneal dialysis patients with anemia were significantly increased compared with that of patients in healthy control group,the results were( 99. 7 ± 36. 7) ng/ml and( 78. 4 ± 20. 6) ng/ml respectively,the difference had statistical significance( P 〈 0. 05); serum hepcidin levels in iron deficiency patients were significantly decreased when compared with that of non-iron deficiency patients,the results were( 69. 4 ±27. 8) ng/ml and( 112. 4 ± 40. 2) ng/ml respectively,the difference had statistical significance( P 〈 0. 01); hepcidin level significantly increased after iron deficiency correction,the result was( 106. 4 ± 34. 3) ng/ml,the difference had statistical significance( P 〈 0. 01); during follow-up,serum hepcidin compared with that at the start of the study,the result
