摘要
目的探讨经鼻内镜泪囊鼻腔吻合术的手术技巧、术后处理和临床效果。方法回顾2015年10月~2017年4月完成的经鼻内镜泪囊鼻腔吻合术患者51例,并对术中采用的吻合泪囊瓣的方法、术后处理及术后效果进行分析和讨论。结果术后随访6个月以上,治愈49例,无效2例,手术成功率96.1%,无相关并发症发生。2例患者术后出现吻合口狭窄,二次手术进行泪道置管。结论经鼻内镜泪囊鼻腔吻合术是治疗慢性泪囊炎首选手段,妥善的处理泪囊瓣以及合理细致的术后局部处理,符合鼻腔生理功能恢复,可以最大程度的避免和减少局部瘢痕增生和骨炎的反应,提高经鼻内镜泪囊鼻腔吻合术的手术成功率。
OBJECTIVE To investigate the surgical approach,techniques and clinical efficacy of dacryocystorhinostomy（DCR） under nasal endoscope in treatment of chronic dacryocystitis. METHODS Fiftyone cases（51 eyes） of chronic dacryocystitis were underwent endoscopic DCR. Their clinical data were analyzed and the ef f icacy of the surgery was evaluated. RESULTS All patients were followed-up for more than half a year.Of the 51 patients, 49 patients were cured, 2 patients were not improved and underwent second operations. The total efficacy rate was 96.1%. There were no complications for all patients. CONCLUSION Endoscopic DCR is the first choice of the chronic dacryocystitis, which has a predictable and favorable success rate. It could be considered appropriate for the initial treatment of nasolacrimal duct obstruction.In addition, postoperative treatment（frequent follow-up,possible use of nasal steroids and frequent cleaning of the granulation tissue） can also increase the success rate of the operation and is at least as important as the operation itself.
