摘要
目的:运用CBCT评价种植体支抗压低下前牙的矫治效果和牙根吸收情况。方法:选取10例因下颌前部牙槽骨垂直向发育过度导致深覆牙合的患者,采用种植体支抗压低下前牙约3~6个月。压低前后拍摄CBCT,通过Romexis影像分析软件评估下前牙压低效果及牙根吸收情况。结果:(1)各组牙齿的Lcr-MP距(阻抗中心至下颌平面的垂直距离)均减小:中切牙平均减小3.26 mm,侧切牙平均减小2.72 mm,尖牙平均减小1.26 mm,差异有统计学意义(P<0.05);(2)各组牙齿的L/MP角(牙体长轴与下颌平面的后上夹角)均增大,差异无统计学意义(P>0.05);(3)各组牙齿的牙齿长度均减小:中切牙平均减小2.75 mm,侧切牙平均减小1.79 mm,差异有统计学意义(P<0.05);尖牙平均减小0.77 mm,差异无统计学意义(P>0.05)。结论:应用种植体支抗压低下前牙治疗深覆牙合的临床效果显著,下前牙压低明显且未出现前牙过度唇倾,但下前牙压低后均有不同程度的牙根吸收。
Objective: To investigate the effect of orthodontic treatment and root resorption of lower anterior teeth intrusion with implant anchorage by cone-beam CT( CBCT). Methods: Ten patients with deep overbite due to mandibular anterior alveolar excess were chosen,implant anchorages were used to intrude lower anterior teeth for three to six months. CBCT were taken before and after intrusion,and the improvement of deep overbite and the root resorption of lower anterior teeth were evaluated by ROmexis image analysis software. Results: The Lcr-MP distance( the vertical distance between the center of resistance and the mandibular plane) were decreased. The central incisors were decreased 3. 26 mm,the lateral incisors were 2. 72 mm and the canines were 1. 26 mm on average,the difference had statistical significance( P〈0. 05). The L/MP angle( the posterior angle between the tooth axis and the mandibular plane) were increased,but the difference had no statistical significance( P〉0. 05). The tooth length were decreased,the central incisors were decreased 2. 75 mm and the lateral incisors were 1. 79 mm on average,the difference had statistical significance( P〈0. 05). The canines were 0. 77 mm on average and the difference had no statistical significance( P〉0. 05). Conclusion: The clinical effect of lower anterior teeth intrusion in treatment of deep overbite with implant anchorage was significant. Lower anterior teeth had not excessive inclination but root resorption were occurred after intrusion.
