摘要
目的探讨血清癌胚抗原(CEA)、细胞角蛋白19片段抗原(CYFRA21-1)、胃泌素释放肽前体(pro-GRP)及糖类抗原125(CA125)在肺癌患者中的表达情况及其诊断学价值。方法选取98例肺癌患者为肺癌组,选取30例肺部良性疾病患者为良性组,选取30例健康体检者为健康组,比较3组研究对象的血清CEA、CYFRA21-1、pro-GRP及CA125水平并进行分析。结果肺癌组的血清CEA、CYFRA21-1、pro-GRP及CA125水平均明显高于良性组和健康组(P﹤0.01);Ⅰ~Ⅱ期肺癌患者血清中CEA、CYFRA21-1、pro-GRP及CA125阳性表达率均明显低于Ⅲ~Ⅳ期肺癌患者(P﹤0.01);血清CEA、CYFRA21-1、pro-GRP及CA125联合检测诊断肺癌的灵敏度、特异度、漏诊率、误诊率分别为82.65%、95.00%、17.35%、5.00%。结论血清CEA、CYFRA21-1、pro-GRP及CA125联合检测诊断肺癌具有较高的灵敏度和特异度,可为临床诊断及治疗提供参考。
Objective To investigate the expressions of serum carcino-embryonic antigen（CEA）, cytokeratin 19 fragment antigen（CYFRA21-1）, pro-gastrin-releasing peptide（pro-GRP） and carbohydrate antigen 125（CA125） in patients with lung cancer and its diagnostic value. Method Ninety-eight patients with lung cancer were assigned to the lung cancer group, 30 patients with benign lung disease were assigned to the benign group and 30 healthy subjects were assigned to the healthy group. The expressions of serum CEA, CYFRA21-1, pro-GRP and CA125 in subjects of the three groups were detected and compared. Result The expression of serum CEA, CYFRA21-1, pro-GRP and CA125 levels in lung cancer group were significantly higher than those in benign group and healthy group（P〈0.01）; the positive expression rates of serum CEA, CYFRA21-1, pro-GRP and CA125 in patients with stage I-II lung cancer were significantly lower than those in patients with stage III-IV lung cancer（P〈0.01）; the sensitivity, specificity, missed diagnosis rate, and misdiagnosis rate of combined detection of serum CEA, CYFRA21-1, pro-GRP, and CA125 in lung cancer were 82.65%,95.00%, 17.35%, 5.00%, respectively. Conclusion Combined detection of serum CEA, CYFRA21-1, pro-GRP and CA125 has a relatively high sensitivity and specificity in the diagnosis of lung cancer, which could be a reference for clinical diagnosis and treatment.
