摘要
目的评价重组改构人肿瘤坏死因子(rmhTNF-NC,天恩福)联合顺铂治疗恶性浆膜腔积液的疗效、安全性和对患者生活质量的影响。方法入组的83例患者中,实验组38例,对照组45例。实验组抽取浆膜腔积液后腔内灌注顺铂50 mg/m^2及rmhTNF-NC200万单位,对照组抽取浆膜腔积液后采用顺铂50 mg/m^2单药灌注。结果实验组38例恶性浆膜腔积液患者治疗有效率为84.21%(32/38),对照组45例恶性浆膜腔积液患者治疗有效率为64.4%(29/45),实验组优于对照组(P<0.01);实验组治疗前后患者KPS评分明显改善(P<0.05);不良反应发生率与对照组相当(P>0.05)。结论 rmhTNF-NC联合顺铂腔内注射治疗恶性浆膜腔积液可取得更加满意的疗效,改善患者生活质量的同时未发生严重的不良反应,值得临床推广。
Objective To evaluate the effect,safety and the impact on patients quality of life with rmhTNF and Cisplatin in the treatment of malignant serous effusion. Methods A total of 83 patients with malignant serous effusion were enrolled into our study. Patients in the experimental group（ n = 38） were perfused with 50 mg/m^2 of cisplatin and 200 million units of rmhTNF.Patients in the control group（ n = 45） were perfused with 50 mg/m^2 of cisplatin. The therapeutic effect,side effects and quality of life were observed. Results For the experimental group,the response rate（ complete response plus partial response） was 78. 9%（ 30/38）,and for the control group,the response rate（ complete response plus partial response） was 73. 3%（ 33/45）. The response rate was significantly difference between the 2 groups（ P 〈 0. 01）,and the side effects was not significantly different（ P 〉0. 05） between the 2 groups. In the experimental group,patients’ quality of life were improved observably after treatment（ P 〈0. 05）. Conclusion RmhTNF combined with cisplatin for malignant serous effusion is affirmed more effectively. The method can improve quality of life,without severe side effects. It is worthy of clinical application.
