摘要
目的:探究高频彩超和常规彩超在诊断睾丸蒂扭转上的临床应用。方法:将60例睾丸蒂扭转患者作为研究对象,对其高频彩超结果及常规彩超结果进行回顾性分析,对比高频彩超及常规彩超在诊断睾丸蒂扭转上的检查结果。结果:高频彩超结果显示,多数患侧睾丸与健侧相比有所增大,其内部表现为低回声或稍强回声,可见分布不均点状回声,边界清晰,阴囊壁有所增厚,有少量或中量鞘膜腔积液。CDFI结果显示患侧睾丸内部血流信号明显减少或消失。在诊断睾丸形态饱满,睾丸内部回声改变以及睾丸内血流正常,血流减少和血流消失上,高频彩超的检出比例(38.33%,18.33%,23.33%,45.00%,71.67%)明显高于常规彩超(15.00%,6.67%,8.33%,25.00%,41.67%,P<0.05)。同时,高频彩超在诊断60例睾丸蒂扭转上的准确率为96.67%,明显大于常规彩超(78.33%,P<0.05)。结论:在诊断睾丸蒂扭转上,高频彩超具有特征性的声像图特点及血流动力学特点,与常规彩超相比误诊率明显减小,能够为其早期诊治提供重要依据。
Objective： To research the clinical application of high frequency color doppler ultrasonography and conventional color doppler ultrasonography in the diagnosis of testicular pedicle torsion. Methods： 60 patients with torsion of testicular pedicle were selected as the research objects. The results of high frequency color doppler ultrasonography andconventional color doppler ultrasonography were analyzed and compared in diagnosis of torsion oftesticular pedicle. Results： High frequency color doppler ultrasonography showed that the affected side testes increased compared with the healthy side,characterizedwith internal hypoechoic or slightly visible echo,uneven distribution of echo,clear boundary and scrotal wall thickening,with little or medium hydrocele. The results of color doppler flow imaging（ CDFI） showed that the internal blood flow signals of the affected side testes decreased or disappeared. In the diagnosis of testicular morphology,internal echo changed with normal testicular blood flow,and blood flow decreased or disappeared. The detection rate of high frequency ultrasound（ 38. 33%,18. 33%,23. 33%,45. 00%,71. 67%） were significantly higher than those of conventional ultrasound（ 15. 00%,6. 67%,8. 33%,25%,41. 67%）（ P〈0. 05）. The accuracy rate of high-frequency color doppler ultrasonography in the diagnosis of torsion was 96. 67%,which was significantly higher than that of conventional color doppler ultrasonography（ 78. 33%,P〈0. 05）. Conclusions： In the diagnosis of testicular pedicle torsion,high frequency color doppler ultrasonography has sonographic characteristics and hemodynamic characteristics. Compared with conventional color doppler ultrasonography,the misdiagnosis rate is obviously reduced,which can provide an important basis for its early diagnosis and treatment.
