摘要
目的观察经皮椎体后凸成形术(PKP)联合中药塌渍法治疗隐匿性骨质疏松性椎体压缩骨折的早期临床疗效。方法 48例(58个椎体)隐匿性骨质疏松性椎体压缩骨折患者,随机分成治疗组和对照组,每组24例。两组患者均采用PKP手术治疗,术后第2天对照组口服阿仑磷酸钠片+钙尔奇D3片+骨化三醇软胶囊治疗,治疗组在对照组基础上加用中药塌渍法治疗。观察两组患者临床疗效及术后疼痛[视觉模拟(VAS)评分]、功能障碍[Oswestry功能障碍指数(ODI)评分]改善情况。结果两组患者手术均顺利完成,无并发症发生。两组患者术后第3天,VAS、ODI评分比较差异无统计学意义(P>0.05);术后第7、14天,VAS、ODI评分比较差异有统计学意义(P<0.05)。结论 PKP联合中药塌渍法治疗隐匿性骨质疏松性椎体压缩骨折在缓解术后早期残留疼痛及改善功能障碍方面效果显著,副作用小,患者依从性高,值得临床推广。
Objective To observe the early clinical effects on occult osteoporotic vertebral fractures treated with percutaneous kyphoplasty(PKP） and the external medicinal liquid application of Chinese herbs. Methods A total of 48 patients of occult osteoporotic vertebral fracture(58 vertebras） were randomized into a treatment group and a control group,24 cases in each one. PKP was adopted in both groups. On the 2 ndday after surgery,the oral administration of alendronate sodium tablets,caltrate D and calcitriol capsules were used in the control group. In the treatment group,on the base of the treatment as the control group,the external medicinal liquid application of Chinese herbs was added. The clinical effects,the post-surgical pain(VAS score） and ODI score were observed in the two groups. Results All of the cases had the successful surgery in the two groups,without complications. On the 3 rd day after surgery,the differences were not significant in VAS and ODI between the two groups(P 〉 0. 05）. On the 7 th and 14 thdays after surgery,the differences were significant between the two groups(P〈 0. 05）. Conclusion Combined with the external medicinal liquid application of Chinese herbs,PKP achieves the significant effects on the post-surgical residual pain and the improvements in functional impairments of occult osteoporotic vertebral fractures. This treatment is less in side effects and high in patient＇ s compliance.Hence,it deserves to be promoted in clinical treatment.
