摘要
目的探讨子宫内膜癌患者辅助化疗期间认知心理治疗对心理状态及疼痛的影响。方法选择2016年9月至2017年6月在天门市第一人民医院治疗的子宫内膜癌Ⅱ期患者80例作为研究对象,按随机数字表法分为观察组和对照组各40例。两组均给予辅助化疗(TC方案),对照组给予常规护理干预,观察组在常规护理基础上联合认知心理治疗,治疗前后对所有患者采用汉密尔顿抑郁量表(Hamilton Rating Scale for Depression,HRSD)、汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)进行心理状态评分,并采用视觉模拟评分法(visual analogue scale,VAS)进行患者疼痛程度评分,对比两组患者干预前后的心理状态及疼痛程度变化。结果两组患者在HRSD、HAMA、VAS评分方面均明显低于治疗前,差异有统计学意义(P<0.05),且治疗后观察组评分低于对照组,差异有统计学意义(P<0.05)。结论子宫内膜癌辅助化疗期间认知心理治疗干预对患者心理状态及疼痛程度有显著改善作用,值得临床推广。
Objective To investigate the effect of cognitive psychotherapy on the psychological status and pain degree in patients with endometrial cancer during adjuvant chemotherapy. Methods 80 patients with stage Ⅱ endometrial cancer in Tianmen First People＇s Hospital from September 2016 to June 2017 were collected and randomly divided into observation group and control group,each with 40 cases. Both groups were given adjuvant chemotherapy（ TC）,the control group was given routine nursing intervention,and the observation group was given cognitive psychotherapy combined with routine care. The score of Hamilton rating scale for depression（ HAMA）,Hamilton Anxiety Scale（ HRSD） and visual analogue scale（ VAS） of two groups were compared before and after treatment. Results The scores of HRSD、HAMA and VAS of two groups were significantly lower than those before treatment（ P〈0. 05）,and the score of observation group was lower than the control group after treatment,and the difference was statistically significant（ P〈0. 05）. Conclution Cognitive psychotherapy has significant effect on psychological status and pain improvement of patients with endometrial cancer during adjuvant chemotherapy,and worthy of promoting in clinic.
