摘要
目的探讨桑珠滋阴口服液治疗干燥综合征(SS)合并间质性肺炎的疗效及可能机制。方法收集我院2014年1月至2017年10月收治确诊的SS合并间质性肺炎患者109例,根据入院顺序随机分为对照组(n=52)和观察组(n=57)。对照组给予甲泼尼龙(120mg/d,静脉滴注,3d)+人免疫球蛋白(10g/d,静脉注射,3d)+环磷酰胺(0.4~0.8g/月,静脉滴注,连续3个月),观察组在对照组基础上给予桑珠滋阴口服液(10ml/次,一日2次,连续3个月)。观察患者口干及眼干症状改善情况,记录患者治疗前后的肺功能,评价两组疗效,并检测T淋巴细胞亚群,观察两组治疗期间与用药相关的不良反应发生情况。结果与治疗前相比,两组治疗后最大肺活量、一秒钟用力呼气容积占预计值百分比、最大通气量、25%肺活量的最大呼气流量、肺活量、深吸气量、补呼气量和最大呼气流量等肺功能指标及CD3^+、CD4^+、CD4^+/CD8^+和NK等T淋巴细胞亚群均有不同程度增加(P<0.05或P<0.01);与对照组相比,观察组治疗后肺功能各项指标及淋巴细胞亚群均增加明显(P<0.05)。与对照组相比,观察组总有效率更高(P<0.01)。两组未发生明显的与用药相关的不良反应。结论桑珠滋阴口服液治疗SS合并间质性肺炎疗效佳,其机制可能与改善患者肺功能及提高T淋巴细胞亚群有关。
Objective To investigate the curative effect of Sangzhu Ziyin oral liquid on the Sj？gren＇s syndrome（SS） combined with interstitial pneumonia and its mechanism. Methods A total of 109 cases of SS patients with interstitial pneumonia selected from January 2014 to October 2017 were randomly divided into two groups. The control group（n=52） was given methylprednisolone（120mg/d, ivgtt, 3d） plus human immunoglobulin（10g/d, i.v, 3d） and cyclophosphamide（0.4-0.8g/m, ivgtt, 3 months）. The observation group（n=57） was additionally given Sangzhu Ziyin oral liquid（10ml/times, 2 times a day for 3 months） on the basis of the control group. The efficacy, lung function and T cell subsets before and after treatment in two groups were recorded. The incidence of adverse drug reactions in both two groups were observed as well. Results Compared with before treatment, the lung function indexes（such as forced vital capacity, 1s percentage predicted forced expiratory volume, maximum voluntary ventilation, 25% of maximum midexpiratory flow, vital capacity, inspiratory capacity, expiratory reserve volume, and peak expiratory flow） and T lymphocyte subsets（such as CD3^＋, CD4^＋, CD4^＋/CD8^＋ and NK cells） after treatment in two groups were increased（P〈0.05 or P〈0.01）. After treatment, compared with the control group, the lung function indexes and T lymphocyte subsets were increased in observation group（P〈0.05）. Compared with the control group, the observation group had more total effective rate（P〈0.01）. No serious adverse drug reactions were observed. Conclusion The Sangzhu Ziyin oral liquid can be effectively used as auxiliary to treat SS with interstitial pneumonia to improve the patient＇s lung function and T lymphocyte subsets.
