摘要
目的探讨脐血血气分析与羊水性状评价新生儿缺氧窒息的临床应用价值。方法选取2016年5月~2017年6月本院收治缺氧窒息患儿53例,设为观察组,另选取同期正常新生儿55例设为对照组。所有患儿分娩后即脐动脉血行血气分析,同时观察新生儿出生后羊水性状。结果两组观察组PH、PCO_2、HCO_3、BE与对照组比较,差异具有统计学意义(P<0.05),PO_2比较,差异无统计学意义(P>0.05)。表明新生儿缺氧窒息与脐血血气分析中PH、PCO_2、HCO_3、BE有关联,随着PH与BE值下降,PCO_2与HCO_3值上升,新生儿出现缺氧窒息的发生率也随之提高;脐血血气分析与羊水性状评价以及缺氧窒息人数结果比较,脐血血气分析结果 PH与BE与羊水性状趋势一致,但PCO_2与HCO_3与之趋势相反,羊水性状0度至III度过程,缺氧窒息人数呈现上升趋势。结论采用脐血血气分析与羊水性状评价新生儿缺氧窒息比较符合生理病理的最终诊断,能够客观、准确的反映新生儿出生时的状态,为在临床上是否采取预防和治疗措施提供依据。
Objective To explore the clinical study of the analysis of blood gas and the shape of amniotic fluid in neonatal hypoxia asphyxia.Methods 53 casesin May 2016 to June 2017 in our hospital were anoxia suffocate children, all patients were diagnosed with neonatal asphyxia standard,all children with anoxia suffocate suffocate group and select normal newborn 55 cases of healthy group. The blood gas analysis was conducted by the American i-stat portable blood gas analyzer in 30 min. Observe the water character after birth.Results Two groups of children with umbilical cord blood, blood gas analysis results of each index test results in anoxia suffocate group of PH,PCO2,HCO3, BE compared with healthy group, significant difference statistically significant(P〈0.05),among them,the PCO2 was similar between the two groups was not obvious, no statistical significance(P〉0.05).To suggest that newborns anoxia suffocate and umbilical cord blood,blood gas analysis in PH,PCO2,HCO3,BE linked, with PH value to BE declining,PCO2 and HCO3 value rise, the incidence of newborns in anoxia suffocate increased;Human cord blood gas analysis compared with the number of amniotic fluid properties evaluation and anoxia suffocate,umbilical cord blood blood gas analysis results of PH and BE consistent with the trend of the amniotic fluid properties,but PCO2 and HCO3,contrary to the trend of,amniotic fluid properties 0 degrees to III process, anoxia suffocate present a trend of increasing in number.Conclusion Neonatal umbilical cord blood of blood gas analysis and amniotic fluid properties evaluation was adopted to suffocate comparison conformed to the physiological and pathological diagnosis,eventually able to objectively and accurately reflect the state of the neonatal birth,whether for clinical prevention and treatment measures.
