摘要
目的探讨CT增强扫描与核磁共振成像(MRI)诊断原发性肝癌的临床效果。方法 100例疑似原发性肝癌患者实施术前CT增强扫描,100例疑似原发性肝癌患者实施MRI诊断。比较分析两种不同诊断方式对原发性肝癌诊断的敏感性、特异性、准确率、阳性预测值和阴性预测值。结果 MRI诊断准确率为93.00%,敏感性为96.30%(78/81),特异性为78.95%(15/19),阳性预测值为95.12%(78/82),阴性预测值为83.33%(15/18);CT增强扫描诊断准确率为80.00%,敏感性为92.54%(62/77),特异性为54.55%(18/33),阳性预测值为80.52%(62/77),阴性预测值为78.26%(18/23)。MRI和CT增强扫描对原发性肝癌的诊断敏感性、特异性以及阴性预测值比较,差异无统计学意义(P>0.05)。MRI对原发性肝癌的诊断准确率、阳性预测值高于CT增强扫描,差异有统计学意义(P<0.05)。结论通过CT增强扫描的方式进行诊断,很容易出现漏诊的现象,并且准确率不高;而MRI能够利用信号差来判断病灶和附近组织的关系,有效的提升了诊断的准确率。MRI检查在诊断原发性肝癌方面有很大优势。
Objective To discuss the clinical effect of CT enhanced scan and magnetic resonance imaging(MRI) in the diagnosis of primary liver cancer. Methods A total of 100 suspected primary liver cancer patients underwent CT enhanced scan before operation, and another 100 suspected primary liver cancer patients underwent CT enhanced scan and MRI for diagnosis. The sensitivity, specificity, accuracy positive predictive value and negative predictive value of two different methods for the diagnosis of primary liver cancer were comparatively analyzed. Results MRI had diagnostic accuracy as 93.00%, sensitivity as 96.30%(78/81), specificity as 78.95%(15/19), positive predictive value as 95.12%(78/82), and negative predictive value as 83.33%(15/18). While CT enhanced scan had diagnostic accuracy as 80.00%, sensitivity as 92.54%(62/77), specificity as 54.55%(18/33), positive predictive value as 80.52%(62/77), and negative predictive value as 78.26%(18/23). There was no statistically significant difference in the sensitivity, specificity, and negative predictive value of MRI and CT enhancement scans in primary liver cancer(P〈0.05). MRI had higher diagnostic accuracy and positive predictive value than CT enhancement scans in primary liver cancer, and the difference was statistically significant(P〈0.05). Conclusion Through CT enhanced scan, it is easy to show the phenomenon of missed diagnosis, and the accuracy is not high, but MRI can use the signal difference to determine the relationship between the lesion and the nearby tissue, so as to effectively improve the diagnostic accuracy. MRI examination has a great advantage in the diagnosis of primary liver cancer.
