摘要
目的探讨长柄生物型假体用于高龄股骨颈骨折关节置换术的临床效果。方法选取贵港市人民医院2014年1月—2016年6月高龄股骨颈骨折82例,按照治疗方法分为骨水泥组和生物组,每组41例。骨水泥组采用骨水泥型假体半髋关节置换,生物组采用长柄生物型假体半髋关节置换。观察两组手术时间、术中出血量、下床锻炼时间、住院时间、术后并发症;术前、术后1、3、6个月采用Harris髋关节评分。结果两组手术时间、术中出血量、部分负重时间、住院时间比较差异均无统计学意义(P>0.05)。两组切口感染、深静脉血栓和假体周围骨折发生率比较差异均无统计学意义(P>0.05),骨水泥组心肺事件发生率显著高于生物组(P<0.05)。两组术前、术后1、3、6个月Harris髋关节评分、术后6个月优良率比较差异均无统计学意义(P>0.05)。结论对高龄股骨颈骨折患者采用长柄生物型假体半髋关节置换可获得良好效果,且可减少心肺事件发生率。
Objective To investigate clinical effect of joint replacement by long-stem biological prosthesis in application of elderly patients with femoral neck fracture. Methods A total of 82 elderly patients with femoral neck fractures admitted during January 2014 and June 2016 were divided into bone cement group and biological prosthesis group( n = 41 for each group) according to therapeutic methods. Bone cement group was treated with semi hip replacement by bone cement prosthesis,while biological prosthesis group was treated with semi hip replacement by long-stem biological prosthesis. Operative time,intraoperative bleeding volume,beginning training time of getting out of bed,hospitalization time,postoperative complications and Harris hip joint scores before and in 1 month,3 months and 6 months after operation were observed in two groups. Results There were no significant differences in operative time,intraoperative bleeding volume,partial weight-bearing time and hospitalization time in two groups( P〈0. 05). There were no significant differences in incidence rates of wound infection,deep vein thrombosis and ambient prosthesis fractures in two groups( P〈0. 05),while incidence rate of cardiopulmonary events in bone cement group was significantly higher than that in biological prosthesis group( P〈0. 05). There were no significant differences in Harris hip joint scores before and in 1 month,3 months and 6 months after operation and excellent rate in 6 months after operation in two groups( P〈0. 05).Conclusion Joint replacement by long-stem biological prosthesis in application of elderly patients with femoral neck fracture may achieve good effect and reduce incidence rate of cardiopulmonary events.
