摘要
目的:分析丙泊酚对剖宫产术中欣母沛不良反应的预防效果及对血流动力学的影响。方法:选择我院2017年1月至2017年10月收治的86例剖宫术产妇,按随机数表法分为研究组和对照组,每组43例。两组均予以硬膜外麻醉,于胎儿娩出后静脉滴注10U缩宫素,并于宫体注射250μg欣母沛,研究组即刻予以静脉泵注3mg.kg/min丙泊酚,对照组静脉泵注等量生理盐水。比较两组不良反应,手术前后血流动力学,手术情况。结果:研究组面部潮红、胸闷、恶心呕吐及头晕头痛发生率均低于对照组,结果有统计学差异(P<0.05)。术前,两组心率(HR)、舒张压(DBP)、收缩压(SBP)、呼吸频率(RR)及血氧饱和度(Sp O2)比较无差异(P>0.05);术后,两组HR、DBP、SBP及RR均上升,Sp O2相应下降,研究组变化幅度更小,结果有统计学差异(P<0.05)。两组出血量、拔管时间及手术时间比较无差异(P>0.05)。结论:丙泊酚对剖宫产术中欣母沛的不良反应的预防效果肯定,能够减少产妇的不适感,更有利于血流动力学的平稳,对临床工作有一定应用价值。
Objective: To analyze the preventive effect of propofol on cesarean section hemabate adverse reactions and influence on hemodynamics. Methods: 86 cases of caesarean section were received therapy from January 2017 to October 2017. According to the random table method,those patients were divided into control group and research group,43 cases in each group. Both groups were given epidural anesthesia,intravenous infusion of 10 U oxytocin after childbirth,and in the palace injection of 250μg hemabate. The patients in the study group were immediately given intravenous infusion of 3 mg.kg/min propofol,while the control group was intravenously injected with the same amount of normal saline. The adverse reactions,hemodynamics and operation before and after operation were compared between the two groups. Results: The incidence of flushing,chest tightness,nausea and vomiting,dizziness and headache in the study group were lower than those in the control group( P〈0. 05). Before operation,there was no difference in heart rate( HR),diastolic pressure( DBP),systolic pressure( SBP),respiratory frequency( RR) and oxygen saturation( Sp O2) in two groups( P〉0.05). After operation,two groups of HR,DBP,SBP and RR all increased,Sp O2 decreased correspondingly,and the amplitude of change in the study group was smaller than that in the control group( P〈0.05).There was no difference in bleeding volume,extubation time and operative time between the two groups( P〉0.05). Conclusion: The effect of propofol on prevention of adverse reactions that cesarean section of hemabate,can reduce maternal discomfort. It is more conducive to the stability of hemodynamics,and has certain guiding value for clinical work.
