紫珠提取物联合七氟烷对心肌缺血再灌注损伤的预防效果观察

摘要 目的:观察紫珠提取物联合七氟烷对大鼠心肌缺血再灌注损伤的预防作用。方法:选取健康清洁级SD大鼠37只,采用随机数字表法将其分为假手术组(Sham)、心肌缺血再灌注组(I/R)、七氟烷后处理组(I1)、紫珠提取物联合七氟烷后处理组(I2)。预处理:I2组给予紫珠提取物灌胃处理,Sham组、I/R组、I1组给予等量生理盐水灌胃;2周后建立心肌缺血再灌注损伤模型:Sham组仅分离左冠状动脉前降支,I/R组缺血30 min后恢复灌注,I1和I2组缺血再灌注前1 min开始吸入3%七氟烷15 min。术后3 d,比较各组左心室射血分数(LVEF)、左心室短轴缩短分数(LVFS)、心脏梗死面积、MDA含量、SOD活性和显微结构变化。结果:I2组LVEF及LVFS均明显大于I/R和I1组,心肌梗死面积明显小于I/R和I1组,心肌组织病理改变结果显微状态明显优于I/R和I1组,MDA含量明显低于I/R和I1组,SOD活性明显高于I/R和I1组,差异均有统计学意义(P<0.05)。结论:紫珠提取物联合七氟烷有效预防心肌缺血再灌注损伤的效果优于单用七氟烷的效果。 Objective: To observe effects of Callicarpa nudiflora extract combined with Sevoflurane in prevention of myocardial ischemia reperfusion injury. Methods: 37 healthy and clean SD rats were selected and randomly divided into sham group,myocardial ischemia reperfusion injury( I/R group),Sevoflurane posttreatment group( I1) group,and Callicarpa nudiflora extract combined with Sevoflurane posttreatment group( I2) group according to the random number table method. For pretreatment,the I2 group was given Callicarpa nudiflora extract by gavage,while the sham group,I/R group and I1 group were given same amount of normal saline by gavage. 2 weeks later,the myocardial ischemia reperfusion injury models were established: only the left anterior descending artery of the sham group were separated; in the I/R group,perfusion was resumed 30 min after ischemia; and in the I1 and I2 groups,3% Sevoflurane was inhaled 1 min before ischemia and reperfusion for 15 min. 3 days later,the left ventricular ejection fraction( LVEF),left ventricular short axis shortening fraction( LVFS),infarct size,MDA content,SOD activity and microstructure of the four groups were compared. Results: The LVEF and LVFS in the I2 group were significantly greater than those in the I/R and I1 groups,the myocardial infarct size was significantly lower than those of the I/R and I1 groups,the microscopic state of myocardial histopathological changes was significantly better than those of the I/R and I1 groups,the MDA content was significantly lower than those in the I/R and I1 groups,the SOD activity was significantly higher than in than those in the I/R and I1 groups,and the differences were statistically significant( P〈0. 05). Conclusions: Callicarpa nudiflora extract combined with Sevoflurane in the prevention of myocardial ischemia reperfusion injury is more effective than single Sevoflurane.
