摘要
目的探讨鼻窦炎术后分别采用医用膨胀海绵和纳吸棉进行鼻腔填塞对术后效果的评价。方法选择2014年12月至2015年12月收治的203例双侧慢性鼻窦炎伴鼻息肉患者,采用计算机随机法将患者分为膨胀海绵组101例和纳吸棉组102例。203例患者均在表麻+局麻下行鼻内镜下双侧鼻息肉摘除+全组鼻窦开放术,术毕按照分组分别用膨胀海绵或纳吸棉进行鼻腔填塞。比较两组术后1、6、12、24、48、72 h的VAS评分情况;术后72 h内出血量、鼻腔症状评分;术后效果评价。结果 (1)术后1、6 h两组疼痛评分相似(P>0.05);术后12、24、48、72 h纳吸棉组较膨胀海绵组疼痛症状轻(P<0.05);(2)术后72 h纳吸棉组相比于膨胀海绵组出血量更少、鼻腔症状评分更低(P<0.05);(3)两组患者术后填塞效果比较,差异有统计学意义(P<0.05)。结论两组材料鼻腔填塞均可有效止血,但纳吸棉比膨胀海绵更有利于减轻患者痛苦,减少鼻腔出血,改善鼻腔症状,值得推广。
Objective To evaluate the nasal packing effect of medical expansion sponge and suction cotton after sinusitis surgery.Methods Two hundred and three patients who were diagnosed as bilateral nasal chronic sinusitis with nasal polyps in our hospital from December 2014 to December 2015 were divided into two groups by the computer random method： expansion sponge group（ n =101） and Nano-absorption cotton group（ n = 102）.All the patients were treated with a bilateral nasal polyp excision combined with a full set of sinus operation surgery under nasal endoscope with surface anesthesia and local anesthesia.Then,the patients were filled with medical expansion sponge and Nano-absorption cotton in nasal cavity,respectively.The VAS scoring was analyzed in 1 hour,6,12,24,48,and 72 hours after operation,respectively.Besides,the amount of bleeding and nasal symptom scores was also observed after 72 hours of operation.Results No significant difference in the pain scores after 1 hour and 6 hours between the two groups（ P〉0. 05）. After treatment for 12,24,48,and 72 hours,the swelling pain symptom in the Nano-absorption cotton group was lighter when compared to the expansion sponge group（ P〈0. 05）.Besides,the bleeding and the nasal symptom scores in the Nano-cotton suction group were also less and lower than those in the expansion sponge group after 72 hours of treatment（ P〈0. 05）.Furthermore,there was a significant difference in nasal packing effect between the two groups（ P〈0. 05）.Conclusion Both nasal packing methods could stop bleeding effectively.However,compared to medical expansive sponge,Nano-absorbing cotton provides a better effect to reduce the bleeding after nasal surgery,improves the nasal symptoms and reduces the patient＇s pain significantly.It should be extended in clinic.
