经阴道实时三维超声造影评估子宫输卵管通畅性的准确性分析

Accuracy analysis of transvaginal real-time three-dimensional ultrasound contrast in the evaluation for uterus oviduct patency

摘要 目的分析经阴道实时三维超声造影评估子宫输卵管通畅性的准确性。方法选取80例不孕症患者,所有患者均给予经阴道实时三维超声造影检查,对其输卵管通畅性进行评估,将腹腔镜通染液试验检查结果作为判断金标准,对经阴道实时三维超声造影检查的图像质量、检查准确性、特异性和灵敏度进行评价。结果80例经阴道实时三维超声造影检查图像中,质量为优55例,良16例,差9例,图像质量优良率为88.8%(71/80)。经阴道实时三维超声造影检查子宫榆卵管通畅性的准确率为92.5%(74/80),特异性为98.1%(52/53),敏感度为81.5%(22/27)。在检查费用、检查后2dVAS评分方面及经阴道实时三维超声造影检查均显著优于腹腔镜通染液试验检查(P〈0.05)。结论在对子宫输卵管通畅性进行检查评估时,经阴道实时三维超声造影检查具有非常重要的作用,不但能明确输卵管的走行、结构和阻塞部位,且操作简单,安全性高,值得临床推广。 Objective To analyze the accuracy of transvaginal real-time three-dimensional ultra- sound contrast in the evaluation for uterus oviduct patency. Methods In 80 cases of infertility, all pa- tients were given transvaginal real-time three-dimensional ultrasound contrast examination, evaluate the patency of uterus oviduct. Laparoscope on-line dyeing test results were used as judgement gold standard, evaluated the image quality, examination accuracy, specificity and sensitivity of transvaginal real-time three-dimensional ultrasound contrast examination. Results In the 80 cases, transvaginal real-time three-dimensional ultrasound contrast examination picture had excellent quality in 55 cases, good quality in 16 cases, and poor quality in 9 cases, the excellent rate of image quality was 88.8% (71/80). The accuracy of transvaginal real-time three-dimensional ultrasound contrast examination for uterus oviduct was 92. 5% (74/80), the specificity was 98. 1% (52/53), the sensitivity was 81.5% (22/27). In the terms of inspection charge, VAS score after 2 days of examination, transvaginal real-time three-dimensional ultrasound contrast examination were significantly better than the laparoseope on-line dyeing test ( P 〈 0.05 ). Conclusions In the evaluation for uterus oviduct patency, transvaginal real-time three-dimensional ultrasound contrast has an importment effects. It can not only determine the passage, struc- ture and obstruction of oviduct, but also be easy to operate and safe, it's worth popularizing.
